Overview


We are seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to join our healthcare team. The ideal candidate will have a strong background in patient care, with experience across various clinical settings including long-term care, acute care, and specialized units such as ICU, emergency medicine, and geriatrics. The LPN will play a vital role in providing high-quality nursing services, supporting patient assessments, medication administration, and care planning within a dynamic healthcare environment. This position offers an opportunity to work with diverse patient populations, including those with developmental disabilities, dementia, and complex medical needs.

Responsibilities



Perform comprehensive patient assessments including vital signs, physical examinations, and health histories using EMR/EHR systems such as Epic, Cerner, eClinicalWorks, and Athenahealth. Administer medications and injections accurately following prescribed protocols while adhering to HIPAA regulations and infection control standards. Assist with invasive procedures such as IV infusion, catheterization, venipuncture, and sterile dressing changes using aseptic techniques. Manage airway care and ventilator support for patients requiring respiratory assistance. Conduct diagnostic evaluations including specimen collection/processing and basic medical imaging procedures. Provide specialized care for patients with conditions like Alzheimer's disease, dementia, traumatic injuries (Level I & II trauma centers), infectious diseases, and post-acute recovery needs. Support discharge planning and coordinate with multidisciplinary teams for case management and home & community care services. Maintain accurate medical documentation of patient care activities including CPT coding, ICD coding (ICD-9/10), and detailed medical records. Educate patients and families on health coaching topics such as nutrition, medication management, pain control, and disease prevention. Ensure compliance with all safety protocols including infection control measures and sterile processing procedures. Collaborate effectively within hospital medicine units such as ICU, PICU, neonatal ventilator units, emergency rooms (ER), urgent care clinics, outpatient facilities, hospice settings, and pediatric or neonatal care environments.

Skills



Proficiency in EMR/EHR systems like Epic, Cerner, eClinicalWorks, or Athenahealth. Extensive clinical experience in areas such as critical care (ICU), emergency medicine (ER), geriatrics (including Alzheimer's & memory care), pediatrics (including neonatal ventilator management), hospice care, dialysis support, and post-acute settings. Strong knowledge of medical terminology, physiology, anatomy, pharmacology related to medication administration and injection techniques. Skilled in performing physical examinations, patient assessment strategies including triage and vital signs monitoring. Ability to manage complex cases involving acute pain management, infectious disease control, ventilator support, catheterization procedures (including urinary & cardiac catheterization), suturing skills, and spinal taps. Familiarity with ICD-9/10 coding standards for accurate documentation; CPT coding knowledge is preferred. Excellent communication skills for working with diverse populations including children with developmental disabilities or special needs. Knowledge of infection control practices and aseptic techniques to prevent cross-contamination in sterile environments. Ability to work independently or collaboratively within multidisciplinary teams in various healthcare settings such as hospitals (including Level I Trauma Centers), clinics or home health environments.
