Job summary:
Under the direction of the Charge Registered Nurse, the Licensed Practical Nurse (LPN) leads the program team in the delivery of resident-centered care and services on a shift-by-shift basis and in accordance with the program goals and processes. The Licensed Practical Nurse directs program team activities, liaises with residents, families and the health authority, supports and mentors team members, makes recommendations for improved systems and processes and continuously assesses the effectiveness of care delivery in support of achieving program goals.
Provides leadership and direction for team members involved in the provision of resident care by mentoring and role modelling, fostering and promoting new initiatives; interpreting, developing and implementing care related policies, procedures and clinical protocols. Delivers comprehensive professional nursing care consistent with the care plan, nursing assessments and resident needs;
Participates in decision-making relating to matters of health and safety, resident care and human resource management. Coordinates the interdisciplinary team to ensure care conferences are held regularly and address the changing needs of each resident. Coordinates the implementation of changes in care delivery resulting from care conference decision-making;
Ensures that resident rights to privacy, dignity and respect are consistently met throughout care and service delivery. Advocates for residents and supports families in maintaining meaningful relationships with residents. Communicates with residents and family members concerning care issues, needs, problems and/or conflicts. Effects positive outcomes to complaints and reports unresolved issues to the Director of Care / Registered Nurse .
Facilitates participation in resident/family meetings and chairs/participates in program team meetings and on OH&S accreditation and quality improvement committees;
Ensures resident records are maintained to the required standards by all program staff. Ensures that proper procedures are followed in the handling and storage of confidential resident, staff and/or company information and medications. Ensures adequate supplies are available to fulfill program requirements.
Nursing Practice:
Utilizes standardized assessment tools based on residents' identified care needs and best practices;
Carries out move-in and move-out processes, and resident transfers;
Performs and completes comprehensive assessments;
Processing Physician / NP orders;
Documenting and reporting;
Medication reconciliation;
Care planning;
Advance care planning;
Performs resident assessment prior to medication administration;
Maintains accurate records of drug counts on controlled medications;
Reports adverse reactions and medication errors to the charge Nurse;
In collaboration with the resident, their family, and the interdisciplinary team, develops an individualized care plan to promote health, prevent illness, and provide person-centered care;
Accurately documents health assessments and pertinent clinical information in a timely manner in the resident's electronic health record and/or chart.
Performs other related duties as assigned.
The successful candidate will have...
Practicing license with the BC College of Nurses and Midwives (BCCNM) in good standing;
Standard First Aid and CPR Level C/AED or equivalent;
Provincial Violence Prevention Curriculum (PVPC);
Demonstrated leadership skills and ability to assess situations and defuse crisis incidents;
Is team-oriented, personable, and demonstrates strong critical thinking skills;
Excellent communicator in English (verbal and in writing);
Proficient knowledge of nursing-related care equipment utilization and management;
Ability to organize and prioritize work efficiently under minimal supervision;
Ability to operate related equipment;
Physical ability to carry out the duties of the position.
Please note: As a result of the previous 'Health Care Labour Adjustment (COVID-19) Order' with respect to the previous Single Site Order, the current HEABC hourly wage-levelled rates for publicly funded care homes continue to be funded by the government and are in place until further directions from the Ministry of Health.
