The candidate will provide nursing services for home care clients, including palliative care and coordination in-home respite services. We provide accessible, high quality health services and programs. The Home & Community Care program is based out of the Jeannie Bird Clinic. This position will provide community-based home care nursing services to members living in La Ronge, including three (3) surrounding communities of LLRIB.About the position: The Licensed Practical Nurse must be able to work within their scope of practice to provide nursing care, treatments and procedures. The LPN will also be responsible for evaluating client progress, and updating the clients care plan based on nursing assessment.Duties:xc2xb7Providing nursing care for home care clients as required;xc2xb7Perform Nursing Treatments and Procedures, as ordered by a physician;xc2xb7Evaluating client progress and updating care plan on an on-going nursing assessment;xc2xb7Providing palliative nursing care and coordinating in-home respite services;xc2xb7 Provide case management to coordinate multiple services, ensuring client is receiving safe, effective and appropriate care;xc2xb7Manage client information through an electronic charting systemQualifications:xc2xb7Registered and in good standing with the Saskatchewan Association of Licensed Practical Nurses (SALPN), or eligible to apply;xc2xb7Acute or home care experience is an asset;xc2xb7Must be able to work independently, be self-directed and demonstrate organizational skills;xc2xb7Proficient computer skills and experience using Microsoft Office;xc2xb7Excellent verbal and written communication skills;xc2xb7Excellent problem solving & organizational skills;xc2xb7Excellent interpersonal, & public relations skills;xc2xb7Ability to speak/understand Cree is a definite asset;xc2xb7Must possess a valid Class 5 Drivers License;
