Your Opportunity:The Licensed Practical Nurse position provides comprehensive nursing support to the Indigenous Wellness Clinic. Our multidisciplinary team provides a culturally informed and safe environment for patients and their families to receive low barrier care and work towards their best possible health. Responsibilities include direct patient care; working with patients to assess learning needs then develop and implement culturally informed, holistic (physical, mental, emotional and spiritual) care plans within a multidisciplinary team with the expectation of self-management; and evaluate whether patient learning needs were met and modify plans as needed. Other responsibilities include supporting evidence-based care for chronic disease management, health promotion, screening and other primary health care related issues; collecting, documenting, reporting and monitoring changes in health status using critical judgment skills; and patient education by working one on one or in group settings with patients and/or families. Assist with the Indigenous Diabetes Wellness Program and other specialties' clinics.Description:As a Licensed Practical Nurse (LPN), you are part of an interprofessional health care team utilizing nursing processes, critical thinking, problem solving and decision making skills. You will be guided by your education, experience, demonstrated skill level and AHS policies and procedures. You will play a key role in providing safe, quality patient and family centered care while reflecting the shared vision and values of AHS.

Required Qualifications:Completion of an accredited practical nurse education program. Active or eligible for registration and practice permit with the College of Licensed Practical Nurses of Alberta (CLPNA). Current Basic Cardiac Life Support - Health Care Provider (BCLS-HCP). Additional Required Qualifications:Minimum three years of recent clinical knowledge and experience in a variety of nursing specialties including, but not limited to: acute care, primary care, chronic disease management especially diabetes, palliative care, reproductive and sexual health, mental health and addictions, pediatrics and/or rural nursing. Experience working in an Indigenous community or health environment. Must possess a sound understanding of Indigenous Peoples and the determinants of health. Demonstrated excellent assessment, organizational and patient care skills required. Ability to communicate findings in a concise and accurate manner; Strong interpersonal and problem-solving skills. Depth & breadth of nursing knowledge & complex reasoning reflective of level of education. Strong communication skills with effective negotiation & conflict resolution skills to improve interprofessional patient/community-centered care. Compassionate & caring attitude; conveying respect & dignity of all persons. Experience with working with family physicians and specialists. Must have knowledge of AHS computer programs/systems such as Connect Care, Microsoft Office and Netcare. Must be able to meet the physical demands of the job, including but not limited to: standing, bending, walking; pushing and pulling equipment; and must be able to assist in transporting, lifting and positioning of patients. Only those candidates with a current resume and cover letter will be considered for an interview Preferred Qualifications:Knowledge of and sensitivity around the culture, health issues, treaty rights, jurisdictional issues, historical impact of the urban and rural Indigenous people/communities in Alberta. Ability to speak or understand an Indigenous language (i.e., Cree, Dene). Group facilitation skills and experience would be an asset.
