The LPN Admissions & Care Coordinator supports smooth transitions for new residents, helps maintain independence, and promotes health and wellness. This role includes pre-admission assessments, personalized care planning, coordination of HCA care, and assisting families with funding options. Acting as the main health contact, the LPN ensures residents receive the right care at the right time.
Key Responsibilities:
Complete pre-admission health assessments and move-in planning.
Develop and update individualized care plans.
Coordinate and delegate care to HCAs, ensuring proper supervision.
Support residents and families with health education and advocacy.
Maintain accurate documentation and regulatory compliance.
Collaborate with management, recreation, and support staff for resident well-being.
Qualifications:
Licensed Practical Nurse (CLPNA registration).
3+ years' LPN experience (geriatrics/community health preferred).
Skilled in assessments, care planning, and team coordination.
Strong communication, organizational, and critical thinking skills.
Knowledge of local care services and referral processes.
Additional Requirements:
Criminal Record Check (VSC), TB test, First Aid/CPR, WHMIS.
