Your Opportunity:Reporting to the Care Manager on Seniors South Team-Home Care Program, the Licensed Practical Nurse (LPN) works as a member of an inter-professional health care team. The LPN is responsible and accountable for their own practice according to the standards defined by the College of Licensed Practical Nurses of Alberta. The LPN is responsible to assess, plan, monitor and evaluate in consultation with the RN Case Manager - Short Term & Long Term, the care needs of Home Care clients and their caregivers/families requiring community-based services. The LPN may work in multiple settings such as residential care, assisted living, homecare clinics and in the community. Responsibilities include: Assessing, planning, organizing and evaluating the needs and goals for clients and their caregivers/families for acute or rehabilitative client groups. Performing short-term assessments and provide direct nursing care on a consultative basis. Reviewing client care needs and goals by planning, organizing and evaluating changes to the care plan as required. Collaborating and consulting with the case managers and the inter-professional health care team as required. Conducting regular home visits. Identifying clients at risk and providing appropriate nursing intervention or advocacy as required. You will be required to operate a laptop computer and mobile phone, and for manual handling (lifting, lowering, carrying, pushing and pulling) as a regular part of the job. For example: lifting of approximately 30 pounds in and out of a vehicle on a regular basis and carrying this weight up and down stairs throughout the day is common within this job.Description:As a Licensed Practical Nurse (LPN), you are part of an interprofessional health care team utilizing nursing processes, critical thinking, problem solving and decision making skills. You will be guided by your education, experience, demonstrated skill level and AHS policies and procedures. You will play a key role in providing safe, quality patient and family centered care while reflecting the shared vision and values of AHS.

Required Qualifications:Completion of an accredited practical nurse education program. Active or eligible for registration and practice permit with the College of Licensed Practical Nurses of Alberta (CLPNA). Current Basic Cardiac Life Support - Health Care Provider (BCLS-HCP). Additional Required Qualifications:Please note: This position is an on-site position. Staff must report to the office daily. There is no opportunity to work from home. Successful completion of a recognized Practical Nursing educational program (diploma or certificate in practical nursing). 3 years of experience working in Home Care or related Community setting required. Completion of a Medication Administration Course for LPN's Minimum. A keen interest in wound care and a willingness to acquire expertise through education & training. Excellent interpersonal, communication and organizational skills & is a team player. Physically able to perform work related tasks and duties. Preferred Qualifications:Proficient with the use of laptop, mobile phone and Microsoft office applications. Experience with Connect Care.
