Your Opportunity:

Reporting to the Area/Care Manager on the Team: Integrated Home Care Program, the Health Care Aide Supervisor, works as a member of an inter-professional health care team. The Health Care Aide Supervisor, primarily responsible for the supervision of Health Care Aides, is responsible and accountable for their own practice according to the standards defined by the College of Licensed Practical Nurses of Alberta. The Health Care Aide Supervisor is responsible for assessing, planning, monitoring and evaluating, in consultation with the Registered Nurse Case Manager- Short Team & Long Term, the care needs of Home Care clients and their caregivers/families requiring community-based services. The Health Care Aide Supervisor may also work in multiple settings such as residential care, assisted living, home care clinics and in the community. The Health Care Aide Supervisor position may also work for Claresholm and Vulcan sites. Primary site: Claresholm Secondary site: Vulcan. At of both.

As a Licensed Practical Nurse (LPN), you are part of an interprofessional health care team utilizing nursing processes, critical thinking, problem solving and decision making skills. You will be guided by your education, experience, demonstrated skill level and AHS policies and procedures. You will play a key role in providing safe, quality patient and family centered care while reflecting the shared vision and values of AHS.

Required Qualifications:

Completion of an accredited Licensed Practical Nurse education program. Active or eligible for registration and practice permit with the College of Licensed Practical Nurses of Alberta (CLPNA). Current Basic Cardiac Life Support - Health Care Provider (BCLS-HCP). Additional Required Qualifications:

Assessing, planning, organizing and evaluating the needs and goals of clients and their caregivers/families for acute or rehabilitative client groups. Collaborating and consulting with the case managers and the interprofessional health care team. Completion of a Medication Administration Course for Licensed Practical Nurses. Conducting home visits. Identifying clients at risk and providing appropriate nursing intervention or advocacy. Performing short-term assessments and providing direct nursing care on a consultative basis. Recent work experience as a Licensed Practical Nurse and experience supervising a team. Reviewing client care needs and goals by planning, organizing and evaluating changes to the care plan. The successful candidate may be assigned to provide direct nursing care to clients in a client's home. They will work with direction from a Registered Nurse. This is an in-scope position reporting to an Area/Care Manager. Preferred Qualifications:

1 to 2 years of experience working in Community / Home Care is a definite asset. A keen interest in wound care and a willingness to acquire expertise through education & training. Excellent interpersonal, communication and organizational skills, and is a team player. Physically able to perform work-related tasks and duties. Supervisory experience.
