Working under the guidance of the Care Coordinator and Registered Nurse, the Licensed Practical Nurse (LPN) assesses, plans, implements, and evaluates nursing care for their work assignment in accordance with competency guidelines within the standards of practice as outlined by the British Columbia College of Nurses and Midwives (BCCNM) current legislative scope of practice and in accordance with the established policies and procedures of George Derby Centre.
Responsibilities and Duties
Assesses, plans, implements and evaluates resident care needs including physiological, psychological, socio-cultural, and spiritual.
Provides direct resident care including administration of medications and treatment.
Reports problems and/or changes to the Registered Nurse or Care Coordinator.
Collaborate with intradisciplinary team to create and modify individualized resident care plans as needed.
Prepare and provide input into interdisciplinary care conferences, medication reviews and other meetings as required.
Provide monthly status updates to primary residents NOK.
When required, assist with admissions, discharges, and transfers of residents/patients.
Maintains paper and electronic documents for accuracy and relevancy, and ensures confidentiality is followed as per policies.
Supervises and provides direction to the Resident Care Aides, including reassignment of workload.
Provides and assists with personal care to residents as required.
Participates in continuous quality improvement, education sessions, resident care conferences, meetings, and committees.
All safety rules and safe work procedures are adhered to at all times.
Reports all hazards, incidents, near misses and workplace injuries to the manager immediately and in writing.
Performs other related duties as assigned.
Qualifications
Education, Training, Experience
Graduation from a recognized program for Practical Nurses or an equivalent combination of education, training, and experience recognized by British Columbia College of Nurses and Midwives (BCCNM).
Maintains Practicing Full Scope Licensure with British Columbia College of Nurses and Midwives (BCCNM)
Maintains Valid First Aid and CPR "A".
Previous experience in a long-term care facility is an asset.

Requirements
Must have an active BCCNM license.
Negative TB test results conducted within the past 5 years
