Summary of Position
The Access 365 Clinic provides referral-based appointments for patients with semi-urgent concerns who cannot see their family doctor or primary care provider and require medical assistance outside regular hours. The clinic ensures continuity of primary health care by supporting PCN members' patients who need medical care within 24 hours but are unable to access their usual primary care provider. The Access 365 clinic team -- including member physicians, nurse practitioners, a registered nurse, licensed practical nurses, medical office assistants, physician lead and program manager(s) -- collaborates closely with internal and external stakeholders to deliver timely care. The team works within a collaborative, team-based model, emphasizing communication, respect and a shared accountability for patient outcomes. Referral sources to the program include 811 Health Link, member physicians and nurse practitioners, Foothills Medical Centre and other Calgary Zone PCNs.
Reporting to the program manager, the Licensed Practical Nurse (LPN) works collaboratively with all clinic staff, multidisciplinary team and physicians in support of patient care. The LPN compassionately provides routine care and patient health assessments, assists doctors and other clinic staff, communicates instructions to patients regarding medication, home-based care and preventative lifestyle changes and other clinical or administrative tasks.
Shift details:
Day shifts: 8:00 a.m. - 3:00 p.m.
Evening shifts: 2:00 p.m. - 9:00 p.m.
Weekend and stat holidays shifts: 8:00 a.m. - 4:30 p.m.
One weekend in a four-week rotation is required
Additional shifts may be available to cover colleagues during absences or peak periods
Key Responsibilities
Key aspects of the position may include, but are not limited to:
Completing full patient assessments and triaging including:
Greeting and rooming the patient
Obtaining and recording vital signs, height, weight and blood pressure when appropriate
Recording a brief and concise history relevant to the patient's chief complaint
Obtaining and processing urine samples when appropriate
Reviewing patient charts daily for completeness, ensuring all pertinent results are on chart and the chart is signed by the practitioner
Chaperoning all physicians during pelvic and breast exams when needed and appropriate
Processing specimens for transport
Using critical thinking to make decisions based on analysis of available information
Performing nursing interventions for which they can manage the client during and after the intervention or has access to resources (O2 administration, dressing changes, injections, nebulizer treatments)
Working in consultation with the PCN health care team to meet care needs of more complex clients
Performing clinic autoclaving
Assisting physicians with opportunistic screens for patients in accordance with the Alberta Screening and Prevention Maneuvers (ASaP)
Maintaining high standards in infection control practices such as:
Using appropriate personal protective equipment when required
Cleaning the room between patients
Maintaining a neat and tidy workspace
Complying with privacy legislation
Complying with Calgary Foothills PCN administrative policies and procedures
Being accountable for their own practice in accordance with College of Licensed Practical Nurses of Alberta (CPLNA) standards and the employment scope of practice
Leading and participating in daily team huddles to support communication, coordination, and continuity across the team
Actively contributing to a supportive and respectful team culture that prioritizes communication, shared learning, coordinated care, and continuous improvement
Collaborating with the health care team as required
Committing to working together in a climate that promotes respect, trust, safety and support for shared decision-making
Collaborating for the benefit of patients, families and providers to create a positive working environment
Collaborating with physicians and other health care providers to provide an integrative approach to care for patients and families
Cross cover and assisting with Medical Office Assistant and administrative duties when necessary
Knowledge, Skills & Abilities
Knowledge of the comprehensive primary health care model and the broad determinants of population health
Knowledge of concepts of health promotion, disease prevention, program planning and individual and group teaching
Communicating with other providers critical information regarding the patient and pertinent issues related to decision-making
Ability to perform specific skills required for the Primary Care licensed practice nurse role in accordance with CLPNA standards of practice
Excellent verbal and written communication skills
Organization and time management skills
Accepts, implements and evaluates change with a positive attitude
Clinical nursing skills to care for medically complex patients
Ability to work well in a team setting
Qualifications
Licensed Practical Nurse (LPN) Diploma required and a current member with CLPNA in good standing
Minimum of one (1) year full clinic scope LPN experience
Immunization Certificate required; or has willingness to complete certification within four (4) months of date of hire
CPR certified considered an asset
Experience in primary care, urgent care, or acute care setting is preferred and will support success in this role
Specialty certifications such as foot care, wound care considered an asset
Employment Requirements
