The Provincial Palliative Care Program is an Ontario Health provincial clinical program within the Primary and Community Based Care Portfolio, Division of Integrated Care. Grounded in ongoing engagement, the Provincial Palliative Care Program collaborates with delivery partners to achieve the Ministry's established strategic priorities for palliative care. The program works closely with Ontario Health Regions, which maintain relationships with local healthcare providers, to implement provincial priorities. The program develops clinical guidance, informs, and/or develops policy related to palliative care, and provides subject matter expertise within Ontario Health, for the government, and partners. Additionally, the program is responsible for health system planning and measurement for palliative care, supports governance bodies, working groups, and expert panels to inform its work, and executes the Ontario Palliative Care Network (OPCN) mandate and priorities in alignment with Ontario Health's role. Established in 2016, the Ontario Palliative Care Network (OPCN) is a partnership of health service providers, community and social support service organizations, health systems planners, as well as patient and family/care partner advisors formed to develop a coordinated and standardized approach for delivering palliative care services in the province. The Lead will be responsible for an overall measurement and monitoring strategy for palliative care in Ontario, working with both internal and external partners. They will provide leadership to team members involved in measurement and evaluation related work, under the direction of the Manager, Provincial Palliative Care Program.
They will:Lead the implementation of the palliative models of care (MOC) implementation measurement and evaluation plans (including measurement and evaluation plans for Indigenous-focused initiatives), along with the development of the provincial measurement strategy for palliative care
Lead monitoring and evaluation of the palliative MOC implementation in identified implementing organizations to ensure the models address barriers to integrated palliative care for individuals and their families and caregivers and improve quality and outcomes.
Collaborate with internal and external partners to ensure measurement and reporting on palliative care aligns with palliative MOC recommendations and expected OH reporting standards
Where Indigenous communities wish to be involved, provide support to Indigenous communities and Indigenous organizations for their measurement efforts in partnership with OH Regions
Here is what you will be doing:Provide palliative care subject matter expertise to build a stronger palliative care ecosystem through the implementation of the Delivery framework.
Responsible for workstreams and deliverables of a high priority, complex, long-term duration project. Manages multiple operational tasks concurrently.
Guide project staff in the achievement of deliverables.
Conduct regularly scheduled status reviews, provides business guidance, proactively identifies deviations from the work plan and implements corrective action.
Develop and present material for review and informational purposes to express complex concepts effectively.
Actively engage with provincial / regional advisory bodies, MOH and subject matter experts to provide strategic advice and thought leadership.
Identify improvement opportunities and escalate key issues/risks to the Group Manager; proposes and executes solutions to address challenges or mitigate risks
Provide measurement related subject matter expertise to support planning, design, implementation and evaluation activities of programs and initiatives associated within its portfolio.
Provide advice and input to the provincial and OH Regional teams on the application of the funding formula and contributes to complex financial decisions related to local system funding and Allocation
Support teams to ensure health service provider and service provider organization adherence to health service accountability agreements.
Coordinate and conduct analysis of local health-care system based on provincial data, community input and other sources for purpose of identifying gaps and establishing planning priorities.
Participate in and contribute to provincial planning activities and conducts effective partner consultation related to assessing impact of planning recommendations and decisions and promoting consensus among partners for action required
Here is what you will need to be successful:
Education and ExperienceA Master's Degree in Health Science, Health Administration, Public Health, Business Administration or Health Informatics or related discipline or equivalent work experience, is required.
6-8 years of overall work experience with performance measurement and evaluation in health care is required, ideally in palliative care
2 years of experience implementing healthcare projects and/or 2 years clinical quality improvement experience preferred
Experience in change management, process development and project management operations is required.
Demonstrated experience with palliative care projects required.
Experience with community health organizations or long-term care homes is an asset.
Knowledge and SkillsA commitment to developing a coordinated and standardized approach for delivering palliative care services for all people in Ontario, regardless of their age or illness
A strong understanding of Ontario's health care system, key provincial priorities for palliative care and broader health system improvement as well
Strong knowledge base on the health related administrative, clinical and financial data available in Ontario and application and use of these data to advance quality improvement
Extensive/Advanced expertise in conducting qualitative analysis using software such as NVivo.
Extensive expertise using data from qualitative and quantitative analyses and other sources to advance QI, including leading completion of literature reviews, jurisdictional scans and other data collection activities for planning, monitoring and evaluation purposes
Advanced knowledge and facility in working with applications and tools used in data and information management (e.g., using MS Excel of other)
Extensive expertise in leading and completing analyses and synthesis of information into summaries, reports, briefs, business cases and/or other documents for various audiences and purposes
Excellent interpersonal and relationship-building skills, with demonstrated experience engaging and working in partnership with diverse partners within the health care sector
Strong communications skills, both oral and written, including effective presentation skills. Internally, communicates with peers throughout the organization to present, discuss information and address risks and issues. Externally communicates with various partners together with manager and/or director when required
Excellent skills and experience in QI, change management and project management and also performance management
Experience in leading working groups comprised of cross-functional staff and clinicians
Able to function independently in setting objectives, prioritizing and managing assigned work (for self and working group), and identifying, addressing and escalating (as needed) risks/issues with manager support as required
Strong problem solving, critical thinking and analytic skills to support effective decision-making and risk/issue management
Able to excel in a fast-paced and objective-based environment
Able to prioritize and effectively anticipate and respond to arising risks/ issues with support and/or escalation to manager when needed
Superior attention to detail with excellent organization and time management skills
Champions the advancement of equity, inclusion, and diversity across Ontario's health care system
Supportive in engaging with First Nations, Indigenous, Metis and urban Indigenous people as they self-determine their own approach to palliative care
Advanced knowledge of Microsoft Outlook, Teams, Word, PowerPoint, and Excel; previous experience with MS Project an asset
