Position Summary
The Kinesiologist works in Mosaic PCN clinics and recreation centre settings to support patients in making positive lifestyle changes related to physical activity.
In clinic settings, the Kinesiologist works with other allied health professionals to provide individualized, team-based care to patients with the goal to improve exercise habits, overall health status, and manage/prevent chronic disease.
In recreation settings, the Kinesiologist will work with other members of the population health team to ensure smooth delivery and operations of population health programs by facilitating group education classes related to exercise, overseeing some day-to-day program operations, and leading group exercise classes for adults with chronic diseases/mobility issues.
Key Responsibilities
Assessment
Assesses the exercise habits of individuals, families, and communities through screening, data collection, ongoing monitoring, and reassessment
Intervention
Educates individuals, families, and communities to assist in applying physical activity principles to improve overall health status using motivational interviewing and behavioural change management
Provides clinical services using a patient-centred approach in both one to one and group settings
Identifies priorities, target dates, and strategies, together with the patients, which contribute to individualized treatment plans and goals
Evaluates the extent to which outcomes have been achieved and recommends alternative treatments as necessary
Consults directly with family physicians and provides referring physicians with concise verbal or written feedback in a timely manner
Develops and delivers exercise education and health promotion programs targeting populations of concern
Facilitates group exercise classes for a diverse population including adaptations based on restrictions due to pain or presence of chronic disease
Inter-professional Collaboration
Works collaboratively with the multidisciplinary team to assess, plan, implement, and evaluate patients' needs to ensure they receive appropriate and optimal care
Documentation
Documents all phases of patient care in the appropriate Electronic Medical Record (EMR) to maintain accurate and timely records, as well as recording the details of clinical interactions in MPCN evaluation systems
Community Engagement
Participates in community activities within the Mosaic PCN catchment area, including health fairs, conferences, and other community events to promote Mosaic programs and services
Professional Development
Identifies and works to achieve own professional development needs which support goals set in performance reviews
Population Health Programming
Participates in the ongoing operations and programming of community-based population health programs
Qualifications & Requirements
Education & Licenses
Bachelor of Science in Kinesiology, Exercise Science, or related field
Minimum designation of Certified Exercise Physiologist (CEP) through the Canadian Society for Exercise Physiology (CSEP) or equivalent through the American College of Sports Medicine (ACSM)
First Aid/CPR Health Care Provider Level C
Experience
Comfortable with counselling patients in a 1-on-1 setting regarding physical activity
Experience working with patients suffering from chronic disease and pain
Able to facilitate educational workshops related to exercise in a group setting (10-15 people)
Experience leading larger group exercise classes (for 20-35 people) with a wide range of abilities
Knowledge, Skills & Abilities
Excellent individual and group teaching skills
Excellent customer service and interpersonal skills
Flexible schedule - able to work some evenings and weekends if needed
Ability to function effectively independently and as a member of an inter-professional team
Knowledge regarding biomechanics, exercise prescription and progression, and adapting exercise for patients with physical impairments
Understanding of pain and chronic disease
Ability to travel between multiple recreation sites and clinics daily
Strong computer skills - fluent with Outlook, Microsoft Excel, Word, Power Point, etc.
Strong oral and written communication skills required
Must be knowledgeable in working with a diverse population facing numerous barriers
Assets
Experience in needs assessments, program development, and evaluation
Experience using an Electronic Medical Records (EMR) system
Motivational interviewing skills
Additional certifications or training in group fitness, biomechanical assessment, or chronic disease management

Working Conditions
Occasional evening and weekend hours required.
Ability to travel on public transportation or valid drivers license with access to own car in good repair is required.
Ability to work at multiple different sites including clinic and recreation settings.
Ability to lead and engage in low-moderate intensity physical activity classes at least 3 times per week.
Ability to move various fitness-related equipment.
Work involves sitting, standing, lifting, demonstrating exercises with/without equipment.
