About the position: The JP Navigator will identify children with unmet needs to facilitate early intervention and timely access to services and resources; Enhance opportunities for the success of at-risk clients and families, Locate resources, Identify options and available supports (JP Group and Individual Applicants Process), Build Case Management Capacity through the development of policies, procedures, and protocols related to screening and assessment, service planning, and monitoring and evaluating of services plans and staff training; Provide a collaborative, client driven system to guide children and their families throughout a comprehensive array of services; Establish multi-disciplinary hubs for families when the services they require involves a number of professionals.

Qualifications: Suitable candidates will have a minimum Bachelor\xe2\x80\x99s Degree in Social Work, Nursing and/or Education or related profession; 3 years\xe2\x80\x99 experience working with First Nations Health, Social and/or Education sectors; A combination of education and relevant experience may be considered; Training and/or experience with assessment/screening tools and case management; Verbal and written communication skills; Ability to travel; Valid driver\xe2\x80\x99s license is required; Ability to speak/understand Cree is an asset.
