St. Lawrence Lodge is a 224 bed Long Term Care Home located on the beautiful shores of the St. Lawrence River, in the City of Brockville.

Job Summary



Reporting to the Director of Care, this role provides oversight and leadership to ensure regulatory compliance and implementation of clinical care standards and best practices. They will ensure the division is meeting obligations in compliance with Long Term Care legislation, divisional policies, standards and IPAC best practices. They will evaluate and assess the care, practices, processes and operations within long-term care homes and work collaboratively to ensure compliance and standards are met and lead continuous quality improvement in nursing clinical program development to ensure best practices in nursing practice and person-centred care.

Duties & Responsibilities



Coordinate clinical policy and program change roll out at home level to support knowledge transfer. Conduct rounds and inspections of the home to ensure safe clinical practices are maintained. Complete appropriate MOHLTC Inspection Protocols and ensures to develop a collaborative action plan to address areas of non-compliance. Review and audit resident charts to ensure MOHLTC compliance standards meet relative to nursing care and clinical programs. Follow-up with nursing team and work collaboratively to address gaps and ensure provision of education to front line staff to support knowledge to practice. Coach, mentor and guide practice advancement and skills development for the nursing leadership team through the development of formal divisional nursing education programs based on identified needs, current research, and divisional priorities. Provide clinical leadership to ensure clinical competency of nursing staff, engages Registered staff in the use of evidence-based best practices, and promotes a culture of continuous clinical/ nursing practice improvement. Demonstrates critical clinical inquiry that supports the development of nursing programs to improves resident care outcomes. Monitor and evaluate critical incident reports; provides direction and advice to homes on actions to minimize risks about high-risk situations in the Division. Collaborate on resident safety and risk, infection prevention and control management issues, and the development of improvement plans to address shortfalls. Monitor home-level clinical performance data quarterly, identifies and evaluates trends, and outcomes; analyzes variances and resource needs; and collaborates with the team to develop and implement action plans for ongoing improvement. Oversee the modernization and transformation of virtual care and other technological solution healthcare records (i.e. Medication Safety Technology). Lead and attend required clinical programs committees as required.

Skills & Qualifications Essential



Current Certificate of Competence in good standing from the College of Nurses of Ontario (CNO). Baccalaureate of Science in Nursing from a recognized University School of Nursing with demonstrated continuous professional nursing leadership development. Certification in Infection Control (CIC) as asset or completion of an IPAC Canada endorsed course and certification in IPAC within three years of being hired with current demonstrated experience with best practice guidelines for infectious disease prevention and control. Extensive experience in the development, implementation, monitoring and evaluation of nursing clinical programs and services. Considerable long-term care clinical experience with a major public or private sector organization in the long-term care, acute care or continuing care/rehabilitation field. Extensive knowledge of relevant legislation, regulations, codes, collective agreements and policies such as the Residents' Bill of Rights, Employment Standards Act, Human Rights Act, Occupational Health and Safety Act, WHMIS, Long-Term Care Homes Act (O.Reg. 79/10), College of Nurses of Ontario and any other pertinent legislation that influences the operation of a long-term care home. Demonstrated experience in clinical quality improvement and change management. Ability to build and maintain effective relationships and partnerships with a diverse range of stakeholders as well as work effectively within inter-professional teams. Effective critical thinking, analytical decision-making, and problem-resolution skills and the ability to distill complex information into clear, concise messages. Ability to manage competing priorities and demands in a rapidly changing environment. Ability to coach, mentor and motivate diverse nursing leadership teams in a unionized environment. Ability to effectively communicate both verbally and in writing, with excellent interpersonal skills. Proficient computer skills with word, excel and electronic document software.

Pay: $93,190.50-$109,860.74 per year
