Job Description


The Interlink Nurse in the Pediatric Hematology & Oncology ambulatory clinic works to provide hospital to community service for children with oncology diagnoses, in collaboration with the Pediatric Oncology Group of Ontario (POGO). The Interlink Nurse will provide family-centered nursing care to children and their families along the continuum of care with an emphasis on the transitions between hospital and community. The Interlink Nurse works as a part of a collaborative, interdisciplinary team, to extend the care of the pediatric oncology patient and family from hospital to home throughout the trajectory of care and provide education to our community partners around pediatric cancer diagnoses, treatment and impact to families. The Interlink nurse also acts as a specialized resource to community based professionals, and advocates for/develops community programs where needed, including supporting siblings, extended family, teachers and principals around a patientxe2x80x99s illness and treatment plan.The Interlink Nurse will help patients and families to navigate the complex healthcare system, conduct home visits to continue assessments, initiate services and support patients and families related to the impact of a cancer diagnosis, including palliative care and to provide bereavement support to family, community and school as appropriate. The interlink Nurse will connect with community and school representatives to support families in coping with and addressing the complex challenges associated with a pediatric cancer diagnosis, treatment, and its sequelae.Qualifications

  • Current Certificate of Registration with the College of Nurses of Ontario as a Registered Nurse
  • A minimum of five (5) years of recent experience in the care of pediatric oncology patients in a dedicated oncology setting
  • Recent experience with primary nursing in an outpatient setting an asset
  • Current APHON Chemotherapy Biotherapy Provider required
  • Certified Pediatric Hematology Oncology Nurse (CPHON) is an asset
  • Current Basic Cardiac Life Support (BCLS) certification and ongoing re-certification required
  • Experience and participation in clinical trials
  • Experience in development of education, programs, and materials with the healthcare team for children with cancer and their families an asset
  • Demonstrated knowledge, skill and ability in health teaching for pediatric patients and their families specific to the needs of the clinic population
  • Demonstrated skills in physical and psychosocial assessment specific to the pediatric oncology population
  • Demonstrated commitment to continuous learning and quality improvement
  • Demonstrated leadership within a healthcare team
  • Demonstrated ability to work both independently and collaboratively within the interdisciplinary health and support team
  • Demonstrated excellence in verbal and written communication skills
  • Experience collaborating with community resources including home care, school, and the Pediatric Oncology Group of Ontario (POGO)
  • Valid G-Class Ontario driverxe2x80x99s license and use of insured vehicle for community travel
