We are seeking a dedicated and compassionate Case Manager to join our team. The Case Manager will play a crucial role in supporting clients by assessing their needs, developing individualized care plans, and coordinating services to ensure the best possible outcomes. This position requires strong interpersonal skills, the ability to work collaboratively with various stakeholders, and a commitment to advocating for clients' needs.
Duties
Conduct comprehensive assessments of clients' needs and circumstances.
Develop and implement personalized care plans in collaboration with clients and their families.
Monitor client progress and adjust care plans as necessary to ensure optimal outcomes.
Facilitate access to community resources, services, and support systems.
Maintain accurate documentation of client interactions, assessments, and progress notes.
Provide guidance and support to clients in navigating complex systems.
Collaborate with healthcare providers, social services, and other agencies to coordinate care.
Supervise and mentor junior staff or interns as needed, fostering a supportive learning environment.
Requirements
Bachelor's degree in Social Work, Psychology, or a related field; Master's degree preferred.
Proven experience in case management or a related field is highly desirable.
Strong communication skills, both verbal and written, with the ability to engage effectively with diverse populations.
Excellent organizational skills with attention to detail.
Ability to work independently as well as part of a team.
Proficiency in using case management software and other relevant technology.
Knowledge of community resources and social services available for clients.
Supervisory experience is a plus but not mandatory.
