will be responsible for providing case management services and facilitation of life skill programming to youth ages 16-24 who are living in a transitional housing program. This position will also include assisting residents with securing longer-term affordable housing outside of SHIP. This role will also require conducting assessments to determine program suitability and to facilitate appropriate referrals. This position will require collaborate with colleagues to determine appropriate transitional plans. This position will operate within a community center and transitional housing program that is focused on providing services to the community and youth who are at risk of homelessness.
Duties & Responsibilities:
Provide counselling supports to promote positive health and independency in the community
Provide specialized case management and community outreach services
Work in partnership with community agencies to coordinate support services
Assistin the implementation and monitoring of residency plan and goals
Assistin developing andmaintainingdaily living and social skills necessary to obtain independency in the community
Facilitate the intake,assessmentand placement of youth into program
Liaise with referring agencies and potential referral sources toassistwith intake into program
Liaise with landlords and other housing service providers toassistyouth with securing permanent and affordable housing accommodations
Act as a liaison between the residents and the Multi-service Teams/Landlords
Facilitation of programming and workshops as it pertains to life skills
Facilitate individual and team approaches in working with youth who may have an identified substance use or mental health concern
You may be required to administer naloxone in response to suspected opioid overdoses, and must complete appropriate training to recognize overdose symptoms, safely administer naloxone via nasal spray or injection, and follow emergency protocols including contacting emergency services. You will receive training within the first eight weeks of employment

Qualifications:
Post-secondary education in Health Services, Social Services, or equivalent
2+ years' experience supporting Youth and/or the Homeless Population or equivalent
A familiarity for supporting individuals with housing isrequired
Demonstrated awareness and belief in the value of harm reduction principles
Excellent assessment, risk management, and consultation skills
A valid Ontario Class "G" Driver's license and a vehicle with adequate insurance coverage
Valid Certification in First Aid & CPR
Training in ASIST and CPI
Working Conditions:
Shifts are typically from Monday - Friday, 7:30-4 or 1:00pm-9:30pm but may vary depending on client and program needs
Averageamount of physical effort isrequired.
Able to sit/stand for prolonged periods of time
Able to answer and communicate telephone calls and general computer use
Able tolift upto 20lbs (product, documents, etc.)
Vehicle travel to sites for community engagement and open doors asrequired.
