Reporting to UA 84 Frederick Street Transitional Program Supervisor, the Intake & Housing Liaison position will be responsible for referral and assessment flow for the 84 Frederick Street Transitional Program.
Their main goal is to support assessments and transitions in and out of the program. They will also provide support to the program staff and staff with ongoing assessment of needs.
Duties and Responsibilities:
Review referrals and conduct clinical assessments to determine service needs

Use standardized assessment tools within a community setting
Develop and maintain a community resource network
Refer to partnering organizations and provide linkages to community supports
Provide streamlined transitions with "warm transfers"
Service co-ordination, follow up and advocacy
Develop outcome and process measures and evaluate
Complete assessments, evaluations and feedback surveys with applicants and/or their families as required
Provide comprehensive, holistic, person and family-centred access to services
Work independently and collaboratively within a multidisciplinary team to carry out mental health recovery and support
Understand and be able to integrate a philosophy of client-centred care into daily responsibilities
Demonstrate a strong commitment to quality care and service delivery based on SHIP's Mission and Values
Ensure appropriate records are kept and ensure that statistical information is maintained as per agency standards
Adhere to agency Health & Safety Program and the Client Safety Framework in the promotion of client safety
Perform additional duties as required/delegated by Management

Qualifications
:Post-secondary education in Social Work, Social Services, Health Services, or equivalent
2+ years experience providing assessments on individuals with mental health challenges
Demonstrated knowledge of serious mental illness, concurrent disorders, and/or dual diagnosis, and experience with intake services preferred
Excellent assessment, risk management, and crisis intervention
Expertise in collaboration with healthcare services
Knowledge of MS Suite (Word, Excel, Power Point and Outlook)
A vehicle, valid driver's license, and adequate insurance coverage
Valid Certification in First Aid & CPR
Training in ASIST and CPI
Reporting Function:
The Intake and Housing Liaison will report directly to the Supervisor, 84 Frederick Street Transitional Program. They will work with both internal and external partners and will interact with all levels within the organization.
They will be a support system, resource, and an advocator for the team and Organization.
Working Conditions and Physical Requirements:
Hours are typically from Monday - Friday, 8:30am - 4:30pm but may vary depending on client needs
Average amount of physical effort is required.
Able to sit/stand for prolonged periods of time
Able to answer and communicate telephone calls and general computer use
Able to lift up to 20lbs (product, documents, etc.)
Vehicle travel to sites for visits/walk-throughs and open doors as required.
