L'Arche Edmonton is looking to welcome an Individual Care Coordinator.

Position Summary




The Individual Care Coordinator collaborates with the Homes and Program Coordinator, House and Day Program Team Leaders and teams, to plan and implement holistic services for all individuals with disabilities in the Homes and Day Program according to the vision of L'Arche Edmonton. The Individual Care Coordinator is committed to the Vision and Mission of L'Arche as outlined in the International Charter of L'Arche and the Identity and Mission of L'Arche. They have a positive attitude that acknowledges the gifts, talents, and passions of persons with disabilities, and an aptitude for service and support.

Responsibilities




In relation to persons with disabilities:

Supports Team Leaders and teams to ensure that the physical, intellectual, emotional, spiritual, and recreational needs of persons with disabilities are documented, accessible and implemented. Conducts and ensures consistency in Individual Service Plans of all persons with disabilities to meet L'Arche and Government standards. Works in collaboration with house teams to ensure development and implementation of ISP goals. Collaborates with Team Leaders and teams to review the needs of persons with disabilities undergoing significant changes. Initiates circles of support based on critical changes where needed. Is a member of the Behavioral Support Committee. Supports teams to develop Behaviour Support Plans and updates as required. Keeps track of all Behaviour Support Plans and related documents. Oversees new referrals and welcoming of new core members. Works with Homes and Program Coordinator as well as House Leaders to facilitate welcoming process and integration of new individuals with disabilities into the community. Assists House Leaders with personal health assessments of people with disabilities. Links House teams with professional health related resources, as needed. Provides guidance in accessing the healthcare system. Attends major or emergency health appointments as needed, especially those related to significant changes in a person health. Works with Teams Leaders and teams to uphold high standards in supporting individuals with disabilities. Communicates with Guardians when there are major changes in a person support needs. Provides information about needs of individuals with disabilities and level of support as required. Ensures that all documentation is filed and up to date as required
In relation to House and Day Program Teams:

Attends House and Day Program meetings as needed. Ensures all documentation is competed and up to date in relation to medical concerns, medical appointments, incident reports, Individual Service Plans, and Behaviour Support Plans. Collects appropriate data monthly.

General:

Oversees Medication Administration training. Leads other training as needed, in collaboration with the Homes and Programs Coordinator and Training, Accreditation and Policy Coordinator. Participates in community committees and other meetings as needed. Is available to carry on call as necessary. Attends ongoing professional development. Fulfill other duties as required.

Preferred Qualifications



2 years of experience working with individuals with disabilities diploma in disabilities studies or equivalent administrative skills Communication skills (verbal, written, presentation, facilitation, listening) research skills organizational skills Understanding of L'Arche Edmonton Policies and Procedures, especially in the areas of responsibilities. Stress management skills Conflict resolution skills Microsoft 365 Apps skills Driver's license

Scheduled 40 hours a week, Monday to Friday, 8:30am - 4:30pm. Evenings and weekends are sometimes required.
