Indigenous Patient Navigator

Mission

Shuswap Band is seeking an Indigenous Patient Navigator, for direct support for families who reside in the Shuswap community. Weekly duties will include a variety of direct family and Elder supports as outlined by the Health Director.

What Will You Do?

This position has a key focus on community mental health and substance use services, also plays a vital role in supporting clients in navigating through various hospital and community services in the region. Additionally, it extends its support to a broad spectrum of health services across the Creston Valley area, encompassing those provided by Interior Health and Indigenous partners. The IPN is committed to employing culturally sensitive practices in both relationship building and service delivery, aiming to enhance patient experiences through effective collaboration with interdisciplinary teams within Interior Health and Indigenous Nations across Canada.

Key Responsibilities

Perform duties in accordance with Shuswap Band Policies and Procedures and as directed by Health Director.

Maintain confidentiality on all matters relating to the Shuswap band.

Greet clients, family members, and caregivers in a positive and friendly manner.

Answering inquiries from clients, family members and caregivers using direction when providing requested information.

Advocate for health and safety of clients.

Monthly trip to Cranbrook and other communities to support our Elders with shopping, elder gatherings, traditional workshops to support with isolation barriers.

Provides transportation to medical appointments and participates in discussions and planning relating to the client's health.

Refers clients' to and support their involvement in community services, groups, and activities.

Home visits with Elders in community to prevent isolation and bring needs back to our care team.

Maintain accurate records of care given and health status changes observed in the clients.

Observes and reports clients and or family changes, accidents, and incidents to supervisor.

Works closely with other social development and community staff in Providing the needed level and quality of services to Shuswap Band members.

Weekly uploading of all invoices and coding as approved by Health Director.

Provide bi-monthly or weekly Elder activities as outlined by the Edlers in community. Examples: Bingo's, crib or card game tournaments, beading, etc.

Support with weekly Edler meal preparation and deliveries.

Perform other duties and responsibilities as necessary to perform the position as assigned by the Health Director.

Qualifications



Minimum Grade 10.

Experience as an Indigenous Patient Navigator or Human Service Diploma would be an asset.

Current Occupational First Aid and Food Safe Level 1.

Possess a valid British Columbia drivers' licence and able to submit current drivers abstract and have reliable transportation.

Complete and clear the RCMP Vulnerable Sector Screening.

Skills and Abilities

- Ability to deal with clients and help maintain and restore physical, social and emotional health of individuals and families to improve the quality of life in the home and community.

- Excellent verbal and written communication skills.

- Physical ability to perform duties of the job.

- Good organizational skills.

- Knowledge of Secwepemc culture is an asset with experience in acknowledging cultural differences and supporting diverse needs.

Qualifications and Terms

1-2 years in a child/youth and family employment setting.

Two years or more of related experience working in a healthcare environment

Experience with working in a First Nation community

Working knowledge and understanding of child development and parenting/family systems

Ability to establish and maintain good interpersonal relationships by displaying tact, courtesy, and patience with client's, family members and others involved in the client's' care.
Knowledge of Secwepemc culture is an asset with experience in acknowledging cultural differences and support diverse needs.

Working knowledge of Microsoft word, excel, PowerPoint, and willingness to learn.

Able to provide a cleared RCMP Criminal Record Check including vulnerable criminal record check.

Possess a Drivers License with a clean drivers abstract.

Must be able to work onsite, in Invermere, BC.

Professional group facilitation skills and ability to bring in speakers for a variety of topics as outlined by interests of the youth and families.

Must be physically capable of performing all duties as outlined by Health Director and Social Worker.

Job Type: Full-time

Pay: $28.00-$32.00 per hour

Expected hours: 32.5 per week
