As an Indigenous Patient Navigator with Vancouver Coastal Health you will:

  • Provide leadership, supervision, and guidance to designated staff and the interdisciplinary team by integrating clinical expertise and a specialized knowledge of Indigenous wellness and traditional healing approaches within the context of family and patient centered care.
  • Promote, implement, and evaluate education programs, resources, and research using knowledge of Indigenous world views, culturally safe care, and adult learning principles.
  • Participate in quality improvement initiatives by evaluating the care provided to patients/clients and their families/caregivers through methods such as assisting and observing staff, reviewing/auditing relevant documentation and conducting interviews to investigate clinical incidents.
  • Act as a resource, supporting staff training and development to ensure delivery of efficient, high standard care for Indigenous patients and their families.
  • Assist the Manager with performing administrative duties related to staffing, scheduling, and clinical research activities and education.
  • Coordinate Indigenous Cultural Safety education and program delivery for the interdisciplinary clinical teams working in in-patient and out-patient settings. Collaborates with interdisciplinary team members, the leadership team, and other health care providers/professionals to achieve excellence in Indigenous patient and family centre care.
Qualifications

Education & Experience
  • Current practicing registration as a Registered Nurse or Registered Psychiatric Nurse with the British Columbia College of Nurses and Midwives (BCCNM).
  • Three (3) years\' recent, related experience in a clinical setting, including one (1) year\'s related administrative/supervisory experience and one (1) year\xe2\x80\x99s related teaching experience and/or group facilitation or an equivalent combination of education, training and experience.
  • Valid BC Driver\xe2\x80\x99s License required as local area travel may require use of own vehicle.
Knowledge & Abilities
  • Comprehensive knowledge of Indigenous healing practices, wellness and perspectives of health.
  • Knowledge of evidence informed clinical practice related to clinical area(s).
  • Knowledge of hospital policies, procedures and standards of care.
  • Knowledge of other health care disciplines and their role in patient care.
  • Knowledge of evidence based nursing practice in the care of clients and families in need of emergency care.
  • Demonstrated ability to teach, demonstrate, facilitate and coach.
  • Demonstrated ability to develop and deliver educational materials for staff/patients/clients and families based on and integrating clinical education and Indigenous education theories, principles and best practices.
  • Demonstrated ability to communicate effectively with co-workers, physicians, other health care staff and clients and their families, either one-on-one and in groups.
  • Demonstrated ability to function as an effective team member, leader, and facilitator.
  • Demonstrated ability to provide leadership and work direction.
  • Demonstrated ability to promote a creative learning environment.
  • Demonstrated ability to establish workload priorities in collaboration with others.
  • Demonstrated ability to provide effective consultation.
  • Demonstrated ability to work independently and in collaboration with others.
  • Skill in use of equipment and techniques appropriate to treatment of clients requiring in-patient care.
  • Demonstrated ability to deal with and/or guide others in resolution of conflict issues.
  • Demonstrated ability to function effectively and efficiently in emergent situations and guide others in order to deliver quality patient care.
  • Demonstrated skill in CPR techniques.
  • Basic computer literacy to operate a computerized patient care information system.
  • Physical ability to perform the duties and responsibilities of the position.
