There are over 40,000 Indigenous people in our Nation's Capital, and Wabano serves more than 15,000 of them each year. While health care is our foundation, Wabano is also a cultural hub for Indigenous people in the National Capital Region--bringing together traditional knowledge, community wellness, and accessible health services. We are seeking an experienced and motivated Indigenous Health & Outreach Lead to guide and expand our Primary Care Mobile Medical Van program and related outreach initiatives. This role will ensure services are responsive to community needs, grounded in culture, and aligned with Wabano's mission to improve Indigenous health outcomes through respect, relationship, and innovation.


RESPONSIBILITIES

Reporting to the Director of the Community Care & Navigation Department, the Indigenous Health & Outreach Lead will coordinate outreach programs, manage the Primary Care Mobile Medical Van, and build partnerships that strengthen community access to holistic health services.


Program Coordination and Operations Coordinate the Primary Care Mobile Medical Van schedule, staff, and community networking. Monitor program performance, develop reports, and identify opportunities for continuous improvement. Ensure seamless, efficient, and culturally safe service delivery across mobile and community sites. Review and communicate medical directives for the Primary Care Mobile to ensure team understanding and compliance. Community Mapping and Outreach Use data, community input, and local partnerships to identify Indigenous populations and service gaps. Target outreach efforts to areas with unmet needs to enhance access to care. Build strong partnerships with Indigenous organizations, health agencies, and community leaders. Community Engagement and Indigenous Health Promotion Engage Elders, Knowledge Keepers, and community members to identify health priorities and barriers. Provide guidance on culturally grounded health promotion strategies. Develop health education and communication materials from an Indigenous lens. Partnerships and Representation Represent Wabano at Ontario Health Team and community health planning meetings. Strengthen relationships with hospitals, health networks, and partner organizations. Provide updates, reports, and recommendations to senior leadership and the CEO. Promote culturally safe practices and advocate for systemic improvements in Indigenous health. Leadership and Supervision Supervise and support the Primary Care Outreach team. Foster a positive, collaborative team environment grounded in Indigenous values of respect, reciprocity, and community care. Provide mentorship, coaching, and performance feedback to staff.

REQUIRED QUALIFICATIONS

Degree or diploma in Health Sciences, Public Health, Indigenous Studies, Social Work, or a related field; or an equivalent combination of education and experience. Minimum 2 years of experience in health program coordination, outreach, or management. Proven experience working with Indigenous communities, organizations, and leadership. Strong understanding of Indigenous health determinants, cultural safety, and traditional wellness approaches. Excellent communication, relationship-building, and community engagement skills. Experience supervising multidisciplinary teams. Valid Driver's License and ability to travel to outreach sites.

SALARY RANGE

$78,000 - $81,000 plus benefits
