Salary range for the position is $34.80 to $50.03 hourly. Interior Health establishes salaries within the minimum and maximum of the salary range based on consideration of the qualifications, experience of the applicant, and an internal equity review of the salaries of other employees.
How will you create an impact?
In accordance with the established vision and values of the organization, the Indigenous Care Coordinator (ICC) works under the direction of the Manager or Director, providing services to Indigenous Individuals as part of the Interdisciplinary Care Team to ensure the provision of care is culturally safe and client centered. The ICC works with Indigenous people located in the assigned Interior Health (IH) service area across varying sectors of health (e.g., acute care, residential services, mental health, public health, and home health services), depending on the local service area.
What you will work on:
• As a member of the direct care team providing care to Indigenous patients, supports positive relationships between Indigenous individuals/family and Interior Health care providers.
• Promotes safe, inclusive, respectful and productive access to care for Indigenous individuals throughout their care journey.
• Supports the exchange of information and assists in problem solving for Indigenous individuals and families in health care settings.
• Advises the care team in the consideration and accommodation of Indigenous individuals' approaches to wellness and care as part of the care plan, including access to Elders, sacred spaces, and care requests such as smudging.
• Advocates for case specific cultural considerations and requirements.
• Counsels Indigenous individuals and families in community options and facilitates outreach to enable community connection with Indigenous support networks and services.
• Collaborates with Clinical Care Team members in developing discharge plans.
• Maintains and updates appropriate documentation and records related to the provision of client care.
• In alignment with IH's Occupational Health & Safety Program, employees shall adhere to all Occupational Health and Safety policies and procedures at all times and attend all required training. Employees are responsible to report any identified hazards, unsafe conditions or incidents to the manager or supervisor immediately.
• Performs other related duties as assigned.
