Join our acute care team as we work to put patients and families at the centre of everything we do, in the role of Income and Coverage Support Assistant.


The Income and Coverage Support Assistant pilot role is a unique cross-site clinical support role aimed at assisting in-patients with financial needs. This role will support patients at both St. Michael's Hospital, as well as, St. Joseph's Health Centre and is primarily designed to support patients in obtaining OHIP and/or other healthcare coverage. This new role will ultimately aid patients in receiving the healthcare they need and in the expedition of safe and timely discharges. The role will work closely with Social Work, Transition Planning and Finance teams.


Duties & Responsibilities:



Develops and maintains up-to-date detailed knowledge of financial issues and financial services available to low-income and/or uninsured patients. Collaborates closely with Transition Planning Teams, Social Workers and Finance to identify, assess and investigate eligibility for various funding/financial supports and problem solves options for income subsidies and alternatives. Provides one-to-one supports regarding the acquisition of required documentation for eligibility around financial supports. Assists patients to obtain new/replacement documents: Health Cards, Birth Certificates, SIN Cards, etc. Assists patients to obtain OHIP coverage where eligible and to access their financial institutions where barriers exist such as lost or stolen cards. Accompanies patients to banks, OHIP offices, ID clinics or other financial-related support appointments when necessary. Supports patients on Interim Federal Health re advocacy and collaboration with physicians to determine expiry dates and with requesting extensions. Assists with applying for financial assistance such as: OW, ODSP, GIS, OAS/CPP, CPP Disability Pension. Assists in obtaining funding for medical equipment required for patient discharges. Maintains strong partnerships with ID clinics and facilitates appropriate referrals for patients requiring ID in order to obtain coverage and/or facilitate onsite ID clinics as required.

QUALIFICATIONS

:


Minimum Social Service Worker diploma or equivalent education and experience Registration in good standing with OCSWSSW preferred Demonstrated knowledge of financial matters affecting uninsured and/or low-income people, including knowledge in the following areas: eligibility criteria for OHIP, interim federal health, federal and provincial income security programs and policies, the Canadian and Ontario tax systems, access to financial services, assets and savings vehicles, and other income support programs such as WSIB and private insurance companies. The ability to develop and maintain up to date knowledge in these areas. Experience in providing one-on-one supports (including problem-solving) to individuals, with diversity in ethnicity/language, income, education, ability, and mental health status. Willingness to travel between St. Joseph's Health Centre and St. Michael's Hospital and to escort patients to any necessary appointments. Demonstrated ability to work effectively in a multi-disciplinary team environment. Experience in a hospital setting is preferred Experience in building relationships with community partners that are outside the health care system Excellent communication, critical thinking and interpersonal skills Proven flexibility and adaptability in a fast-paced environment Excellent time management, organizational and prioritization skills as relates to patient care Current BLS (or willingness to complete within probationary period)
