Responsibilities
Conduct client assessments to identify needs and develop individualized support plans
Maintain detailed records of client interactions, case notes, and service documentation using databases and record management systems
Coordinate with internal teams and external agencies to deliver appropriate services and resources
Prepare reports, presentations, and correspondence utilizing Microsoft Office Suite (Word, PowerPoint, Excel)
Manage case files systematically, ensuring compliance with organizational policies and confidentiality standards
Facilitate communication between clients and service providers through effective negotiation and customer service skills
Monitor client progress and adjust support plans as necessary, demonstrating strong analysis skills
Organize meetings, schedule appointments, and manage time efficiently to meet deadlines
Participate in team meetings and contribute to project management efforts using tools like Microsoft Project or SharePoint
Assist in procurement processes when needed, including filing and record keeping
