Your Opportunity:



As a Hub Site Coordinator, will provide comprehensive administrative and logistic support to our healthcare professionals in a team setting.

The Hamilton Family Health Team (HFHT) is seeking a full-time Hub Site Coordinator 1.0 FTE.

Roles and Responsibilities:



Ensuring the efficient and smooth operation of HFHT Hub site/office,

Managing administrative tasks; overseeing the overall flow of the office environment

Booking patient appointments, greeting patients, and answering the HFHT phone line

Liaising with patients and families in a professional and compassionate manner while maintaining privacy and confidentiality

Maintaining patient records by obtaining, recording, and updating personal information

Scanning and faxing patient information and consultation reports

Liaising with practice-based primary care teams

Overseeing and monitoring room bookings and scheduling

Other duties as required

Qualifications and Skills:



BA or diploma in Medical Office Administration or equivalent Exceptional customer service, communication and organizational skills Excellent interpersonal skills, with proven ability to work well in a team environment Demonstrated problem solving skills and initiative Proficient with Microsoft Office Must be able to multi-task and prioritize while working within a busy environment Experience with EMR (Practice Solutions Suite) an asset Previous experience in a clinical setting an asset
