Job Description

Four Counties Addiction Services Team (Fourcast) is a community-based addiction treatment service provider that strives to provide the highest quality of evidence-based addiction treatment and case management services. Fourcast offers services across 5 sites in the region. The community treatment program staff work in-office and community settings and are often co-located and integrated with other organizations to reduce barriers for those accessing services.

About The Position...



We are looking for an experienced individual for this permanent position working Mondays to Fridays 8:30am - 4:30pm.

Fourcast's Addiction Supportive Housing Program supports those experiencing homelessness through transitional and/or permanent housing solutions and supports them on their path to independent housing. The program works from a Housing First approach and uses an evidence-based case management model that supports housing stability.

The Housing Support Worker (HSW) supports the implementation of housing treatment plans through an assertive engagement approach. The HSW will foster personal growth and skill development for service users through advocacy, coaching and connection with services and resources within the communities.

What We're Looking For:



Requires a community college diploma or an equivalent combination of work experience and education in a related social services field. Minimum of 2-4 years experience in a community-based organization working with adults with mental illness and/or dealing with addictions in the community. Experience with crisis intervention, advocacy, mediation and conflict resolution. Demonstrated experience working with people with serious mental illness and complex needs. Valid Ontario Class G drivers license. Valid First Aid/CPR. Non-Violent Crisis Intervention (NVCI) certificate an asset. Certification in GAIN assessment an asset. Applied Suicide Intervention Skills Training an asset. Satisfactory Vulnerable Sector Check is required.
