Hospital Discharge Social Worker (Part-Time, Casual) -- $60/hour

We are seeking an experienced Hospital Discharge Social Worker to join our team on a part-time basis. The ideal candidate has hospital experience in discharge planning and understands the importance of safe, well-coordinated transitions of care. This role offers flexible hours.

Key Responsibilities:

Support patients and families in planning for treatment. Assess patients' needs and connect them with rehabilitation programs. Assist with applications for benefits, and other support services. Communicate effectively with other stakeholders to ensure continuity of care. Maintain accurate documentation in accordance with Q Medical standards.

Qualifications and skills:

Master's degree in Social Work (MSW) from an accredited program. Registered with the Ontario College of Social Workers and Social Service Workers (OCSWSSW). Minimum 2 years of experience in hospital discharge planning. Knowledge of Ontario healthcare system and community resources. Strong interpersonal, communication, and advocacy skills. Ability to work independently while collaborating within a multidisciplinary team.
