About the Organization

Heart House Hospice is a non-profit, organization providing support to people and their families experiencing life limiting illness and end of life, serving over 1,500 individuals a year in the Mississauga and Brampton areas. Our mission is to provide compassion, support and advocacy that lessens the distress of terminal illness and loss on individuals, families, and the community. Our continuum of support programs includes hospice counselling, spiritual care, HUUG (Help Us Understand Grief) Children\xe2\x80\x99s program, Social Connections, health and wellness education, bereavement and will soon be grown to include, in patient hospice beds within our Hospice Centre.

We are currently looking for a Coordinator for our Social Connections Program. Social Connections is a weekly program that creates a space for palliative individuals to engage in a variety of activities. This program is designed to promote social interaction, improved wellbeing, decrease a sense of isolation and create community. We are looking for a passionate, personable, outgoing, organized individual to continue to develop and grow this program.

This is a part-time position with approximately 12 hours per week. The successful candidate will need to be available part-time on Monday or Tuesday and full time on Wednesday.

About the Opportunity

As the Hospice Social Program Coordinator, you will be responsible for overseeing and ensuring the smooth delivery of weekly day hospice activities available for palliative patients served.

Key Responsibilities:

\xc2\xb7 Reviewing program referrals;

\xc2\xb7 Facilitating meetings with new program participants and ensuring all documentation is completed (i.e. assess, consent, present);

\xc2\xb7 Preparing and circulating monthly program activity schedule to all participants and volunteers;

\xc2\xb7 Coordinating and scheduling programs and education that support the needs of persons served, including facilitating complementary therapy volunteers and treatments and scheduling outside presenters/professionals;

\xc2\xb7 Overseeing the volunteers and students supporting the program;

\xc2\xb7 Facilitating weekly debrief sessions with volunteers;

\xc2\xb7 Reviewing program process and making program improvements as required including referral, assessment, discharge, evaluation, communication process;

\xc2\xb7 Collecting, inputting and reporting monthly program data within Sumac;

\xc2\xb7 Creating a system for soliciting and reporting on feedback obtained from program participants/stakeholders; coordinating input of all feedback, evaluation and survey data;

\xc2\xb7 Maintaining all documentation relevant to the program;

\xc2\xb7 Sharing data with hospice counsellors as required to support person centered care.

Qualifications and Experience

\xc2\xb7 Degree or diploma in related discipline (recreation, social/community service)

or relevant experience;

\xc2\xb7 Strong written and verbal communication skills;

\xc2\xb7 Strong interpersonal skills

\xc2\xb7 Intake/Assessment skills as asset

\xc2\xb7 Knowledge/experience working within a cultural diverse community;

\xc2\xb7 Empathetic, compassionate, respectful and open minded;

\xc2\xb7 Professional and approachable;

\xc2\xb7 Knowledge and experience with scheduling or arranging social/recreational programs;

\xc2\xb7 Comfortable talking about hospice palliative care;

\xc2\xb7 Familiar with complementary therapies;

\xc2\xb7 Previous experience working with volunteers and volunteering;

\xc2\xb7 Ability to work within a team;

\xc2\xb7 Demonstrated organizational and time management skills;

\xc2\xb7 Experience working with computers and databases;

\xc2\xb7 Additional languages an asset;
