What You'll Do:
Deliver individualized, patient-centered hospice or home health care with compassion and excellence.
Assess patient needs, develop and implement tailored care plans.
Collaborate with interdisciplinary healthcare teams for seamless, holistic support.
Maintain clear and accurate documentation to ensure quality and compliance.
Provide emotional and educational support to patients and their loved ones.
Manage a caseload of 50-60 visits per bi-weekly pay period (full-time) or a 40-hour work week, with fewer visits during initial training.
What You'll Need:
Minimum of 2 years of nursing experience in acute care, home health, or hospice.
Canadian citizenship.
Active California Registered Nurse (RN) license (or willingness to obtain).
Valid driver's license.
Availability to start within one month.
