Summary
The Homeless Prevention Worker reports to the Executive Director or designate and works in accordance with the mission and philosophy of Active Support Against Poverty including ASAP's Code of Ethics. Duties and responsibilities include facilitating preventive measures that assist individuals and families who are at immediate risk of homelessness. The Homeless Prevention Worker assists in development of supportive relationships in the community, including with service and housing providers. The Homeless Prevention Worker will participate as a member of a team responsible for the efficient daily operations of Active Support Against Poverty. This position will be required to fill in for other positions within our organization as deemed necessary.
Responsibilities
Responsible for assisting families and individuals who are absolute homeless or at risk of homelessness due to crisis or transition, including; people leaving corrections and hospital systems; women who have experienced violence or are at risk of violence; youth - including those leaving the care system; and people of aboriginal decent.
Responsible for directly engaging clients where they are situated, completing an assessment, developing a case plan, referring to appropriate housing, income assistance, and support services. Where appropriate, accompanying clients to appointments.
Responsible for accurately and timely entry of shelter and prevention program statistics in data base. Responsible for providing or securing reports from the data base. Completing and maintaining related manual records and documentation.
Responsible for; assisting clients working towards being or are currently placed as tenants to maintain their housing and their ability to live independently by providing tenancy support and skills training; providing follow up and ongoing support to tenants as appropriate; building and maintaining relationships with landlords providing housing to clients; and providing education and resources to landlords to support and maintain client tenancies where appropriate.
Involved in linkages and partnerships with other local service providers to ensure optimal service delivery to clients and participating in local and regional initiatives in order to improve the quality of services to people who are homeless or at risk of homelessness.
Assist in providing rental supplements where appropriate and according to established policies and procedures to eligible and screened clients according to the developed guidelines and direction from the Executive Director.
Participates as a team member with other staff to ensure a safe and caring environment by responding to emergency issues and supporting others through methods such as sharing knowledge and information.
Observes clients and their environments to ensure the safety of clients and the premises by reporting unsafe conditions, incidents and other behavior. Dealing with client emergencies in accordance with guidelines and reporting incidents to appropriate staff and management. Diffuses volatile situations through methods such as non-violent crisis intervention techniques. Records observations for communication to other staff and management.
Responsible for front end coverage as required.
All other duties as required.
Qualification/Skills:-
Minimum Grade 12, two (2) years of recent related experience or an equivalent combination of education, training, and experience.
Crisis Intervention Skills Training is an asset.
Successful completion of Criminal Record Check
Basic problem solving skills
Strong Ability to manage stress
Adequate written and oral communication skills necessary
Work Hours:-
Monday to Friday ( 9:00 AM -4:00 PM)
