The Home Visitation Practitioner provides family-centered support through home visits by fostering relationships and connecting families with resources to enhance child development and overall family well-being.
Key Responsibilities:
Conduct Home Visits: Schedule and perform regular visits on an on-going basis in a supportive environment working with a strength-based model.
Family Needs Assessment: Utilize standardized tools to evaluate family strengths and challenges, developing tailored support plans.
Resource Navigation: Stay informed about community resources, connecting families to healthcare, education, and other essential services.
Parent Education: Offer evidence-based guidance on child development, positive discipline, and emotional support for caregivers.
Child Development Monitoring: Screen children for developmental milestones and provide activities to promote healthy growth.
Crisis Intervention: Recognize signs of domestic violence or child neglect, implementing safety plans as needed. Working from a harm reduction and trauma informed lens.
Documentation: Maintain accurate case files and enter data into the organization's system.
Collaboration: Participate in team meetings and work with other professionals involved in family care.
Attend mandatory training and teachings, staff meetings and Bent Arrow events.
Qualification Requirements:
Human Services degree or diploma; equivalencies will be considered.
Experience with providing support to families in their homes is required.
Ability to model and teach appropriate behaviors such as communication, conflict management, problem solving, decision making, positive parenting and education.
Strong interpersonal skills with good oral and written communication.
Knowledge of the Indigenous culture and language is an asset.
Must have own vehicle for home and community visits.
Must have a valid driver's license and willing to obtain a drivers abstract.
Must have 2-million-dollar liability insurance coverage
Must provide a Criminal Record Check with Vulnerable Sector and Intervention Record Check (within a 6-month period from date of completion).
Strong communication and interpersonal skills.
Salary and Benefits:
Salary plus monthly auto allowance.
Paid sick and wellness days after months' probation (pro-rated).
Extended Health Benefits (50/50 cost sharing) after 6 months' employment.
Working approximately 34.5 hours per week.
