We are seeking compassionate and dedicated Support Workers to provide high-quality care and assistance to individuals in various healthcare and community settings. The ideal candidate will have experience working with seniors, people with disabilities, or those requiring specialized care such as dementia or hospice. This role involves supporting clients with daily activities, medication management, and promoting their overall well-being in a respectful and empathetic manner.

Duties
Assist clients with personal care routines including bathing, dressing, and grooming
Support with mobility and transferring, including heavy lifting when necessary
Administer medications following prescribed protocols and maintain accurate documentation
Prepare nutritious meals tailored to individual dietary needs
Provide companionship and emotional support to enhance quality of life
Assist with light housekeeping tasks such as cleaning and laundry to maintain a safe environment
Support clients with activities of daily living (ADLs) and promote independence
Collaborate with healthcare professionals on case management plans for optimal patient care
Document care provided and report any changes in client condition promptly
Participate in ongoing training related to nursing home procedures, dementia care, hospice, and medical terminology
Skills
Prior experience in nursing home, senior care, or home health preferred
Knowledge of caring for individuals with disabilities, dementia, or hospice needs
Ability to administer medications accurately and understand medical terminology
Strong caregiving skills with a compassionate approach to patient care
Excellent communication skills for working effectively with clients, families, and healthcare teams
Physical ability for heavy lifting and assisting with mobility tasks
Familiarity with assisted living environments and case management processes
Ability to handle multiple responsibilities while maintaining attention to detail in cleaning and safety protocols
Empathy, patience, and a genuine desire to improve the lives of others
