Overview

We are seeking compassionate and dedicated Home Support Workers to provide exceptional in-home and community-based care to individuals with diverse needs. This role is vital in promoting independence, safety, and well-being for our clients, many of whom may have disabilities, chronic illnesses, or age-related conditions. The ideal candidate will have experience in senior care, assisted living, or home health services, and possess a genuine desire to make a positive impact on people's lives.

Duties

Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support Monitor patient vital signs and observe for any changes in health status Administer medications and follow care plans as prescribed by healthcare professionals Prepare nutritious meals tailored to individual dietary needs and assist with feeding if necessary Operate equipment such as Hoyer lifts and ensure safe transfer techniques for residents with mobility challenges Maintain accurate documentation in compliance with HIPAA regulations and care plans Provide companionship and emotional support to promote mental well-being Perform light housekeeping tasks including cleaning, laundry, and organizing to maintain a safe living environment Support clients with cognitive impairments such as dementia or Alzheimer's through specialized memory care techniques Collaborate with healthcare teams to ensure comprehensive patient care and safety

Experience

Prior experience working in assisted living facilities, nursing homes, or home health environments preferred Knowledge of patient monitoring, medication administration, and vital signs measurement required Experience working with individuals with developmental disabilities, autism, or dementia highly desirable Certification in CPR and First Aid is preferred; ongoing training in hospice care or hospice support is a plus Familiarity with HIPAA regulations and maintaining patient confidentiality essential Demonstrated ability to assist with complex care needs including heavy lifting and the use of assistive devices like Hoyer lifts Background in social work or long-term care can be advantageous for providing holistic support Compassionate attitude combined with excellent communication skills necessary to work effectively with clients and their families
