Home Share and Community Inclusion are looking for an enthusiastic and fun-loving person to help!
Vital Society is seeking a skilled and compassionate Home Share and Community Inclusion Program Coordinator to join our team. This position is responsible for coordinating high-quality Home Share and Community Inclusion services that reflect person-centered values and meet all provincial and accreditation standards. The Coordinator plays a key role in recruiting, monitoring, and supporting Home Share Providers and staff while ensuring individuals served experience inclusion, safety, and well-being in their homes and communities.
The successful candidate will demonstrate leadership, collaboration, and a strong commitment to fostering belonging, cultural safety, and quality of life for persons served.
Job Duties
Duties will include, but are not limited to:
Home Share Coordination
Recruit, screen, and assess potential Home Share and Respite Providers in accordance with CLBC Standards for the Coordination of Home Sharing and agency policy.
Facilitate the matching process between individuals and Home Share Providers based on compatibility, lifestyle, and support needs.
Monitor contractors through home visits, safety reviews, vulnerability assessments, and annual evaluations to ensure compliance and quality service delivery.
Provide training, coaching, and mentorship to Home Share Providers to support successful, stable home arrangements.
Maintain accurate documentation in Nucleus and other systems in compliance with CLBC and CARF requirements.
Mediate and resolve issues or complaints in collaboration with the Executive Director or Integrated Service Manager.
Participate in crisis response and assist with temporary placements or support planning as needed.
Respond to emergencies and attend meetings and other events that occasionally involve long work days and evening/weekend work. (flexible work schedule)
Community Inclusion Coordination
Oversee the day-to-day operation of Community Inclusion programs, ensuring quality, individualized supports are delivered.
Supervise and support Community Inclusion employees through training, development, and performance evaluation.
Develop and monitor Individualized Service Plans (ISPs) and Personal Service Plans (PSPs) that reflect the person's goals and aspirations.
Coordinate staffing schedules and manage program hours within budget parameters.
Ensure programs meet all contractual, accreditation, and health and safety requirements.
Collaborate with Home Share and Community Inclusion teams to support continuity of care.
Quality Assurance and Continuous Improvement
Promote best practices in accordance with CLBC standards, CARF accreditation, and Vital Society policy.
Maintain accurate and timely documentation, reports, and records.
Support program review, data collection, and quality improvement initiatives.
Uphold confidentiality, ethical practice, and professional accountability at all times.
Required Qualifications
Post-secondary diploma or degree in Social Services, Community Support, or a related field.
Minimum of three (3) years' experience in the community living or developmental disability sector, including coordination or supervisory experience.
Valid Class 5 BC Driver's License and a reliable vehicle with appropriate business insurance (per Vital Society policy).
Current First Aid and CPR Certification.
Successful completion of or willingness to obtain Community Living BC Home Study Certification.
Knowledge of CLBC Standards for Home Sharing and Standards for the Coordination of Home Sharing.
Familiarity with Health and Safety legislation, person-centered planning, and quality assurance systems.
Desired Qualifications
Strong leadership, interpersonal, and communication skills.
Ability to plan, organize, and oversee community-based supports for individuals with diverse needs.
Knowledge of positive behaviour support principles and trauma-informed practices.
Proven ability to work independently while managing multiple priorities.
Demonstrated understanding of cultural safety, inclusion, and the rights of persons served.
Knowledge of CARF standards and CLBC contractual frameworks.
Proficiency in Microsoft Office and case management systems (e.g., Nucleus).
Restrictions & Requirements
Clear Criminal Record Check (Vulnerable Sector).
Valid BC Driver's License with acceptable driving abstract.
Medical note confirming fitness for role, if required.
Influenza vaccination (or masking required during Influenza season)
CLBC Documentation and Privacy Certificates.
Additional internal and external training as identified (e.g., Home Sharing Standards, Adult Guardianship, Licensing Regulations).
