Job Overview

We are seeking a compassionate and dedicated Home Health Care Aide to provide essential support to individuals in need of assistance with daily living activities. The ideal candidate will have a strong background in patient care, with experience in various caregiving settings such as assisted living, nursing homes, and home health environments. This role requires a commitment to enhancing the quality of life for our clients through personalized care and support.

Responsibilities

Assist clients with Activities of Daily Living (ADLs) including bathing, dressing, grooming, and meal preparation Monitor and document vital signs and changes in patient condition Provide companionship and emotional support to clients Administer medications as prescribed and ensure proper medication management Assist with mobility using equipment such as Hoyer lifts Maintain cleanliness of the client's living space, including laundry and light housekeeping tasks Implement care plans tailored to individual client needs, including Alzheimer's and dementia care Ensure compliance with HIPAA regulations and maintain confidentiality of patient information Collaborate with healthcare professionals to deliver comprehensive care

Qualifications

Previous experience in home health care, senior care, or long-term care settings is preferred Knowledge of medical terminology and familiarity with EMR/EHR systems is a plus Certification in CPR and first aid is highly desirable Experience working with individuals with developmental disabilities or special needs is beneficial Strong observational skills for monitoring patient conditions and reporting changes promptly Ability to perform catheter care and assist with hospice care when necessary Excellent communication skills, both verbal and written A compassionate demeanor with a genuine desire to help others
