Duties
Assist clients with Activities of Daily Living (ADLs) such as bathing, dressing, grooming, toileting, and mobility support using tools like Hoyer lifts when necessary.
Monitor patient health by observing vital signs, managing medication administration, and documenting medical records accurately within Electronic Health Record (EHR) systems.
Support clients with meal preparation, light housekeeping, laundry, and cleaning to maintain a safe and comfortable environment.
Provide specialized care for individuals with dementia or Alzheimer's disease through memory care techniques and patience-driven engagement.
Assist with medical procedures including catheter care, wound care, and hospice support while adhering to HIPAA regulations for patient privacy.
Use medical terminology knowledge to accurately document patient progress and communicate effectively with healthcare teams.
Employ skills in working with people with developmental disabilities or disabilities to promote independence and dignity in daily routines.
Requirements
Proven experience in home health or long-term care environments; nursing home experience is highly valued.
Demonstrated ability to observe patients closely and respond promptly to changing needs or emergencies.
Certification in CPR and First Aid is required; additional certifications such as medication administration or hospice care are a plus.
Familiarity with EMR (Electronic Medical Records) or EHR systems for accurate documentation of patient data.
Strong understanding of medical terminology relevant to home health caregiving.
Ability to work independently in live-in employment settings or community-based environments.
