Overview


We are seeking compassionate and dedicated Home Health Care Aides to provide exceptional in-home care to clients with diverse needs. This role involves supporting patients with daily activities, medical needs, and emotional well-being, ensuring they receive respectful and personalized care in their own homes. Ideal candidates will have experience in senior care, long-term care, or assisted living environments and possess a strong commitment to improving quality of life for those they serve.

Responsibilities



Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support Monitor and observe patient conditions, reporting any changes or concerns promptly Administer medications and manage medication records in compliance with HIPAA regulations Operate Electronic Health Record (EHR) and EMR systems accurately for documentation purposes Support patients with memory care needs such as Alzheimer's and dementia care Perform medical procedures such as vital signs measurement, catheter care, and CPR when necessary Use assistive devices like Hoyer lifts to ensure safe patient transfers Prepare nutritious meals and assist with meal planning tailored to dietary restrictions Conduct light housekeeping tasks including cleaning, laundry, and maintaining a safe environment Document medical records accurately and maintain confidentiality per HIPAA standards Provide emotional support and companionship to enhance patient well-being

Requirements



Prior experience in home health, hospice, nursing home, or assisted living settings preferred Knowledge of medical terminology, medication administration, and caregiving best practices Certification in CPR is required; additional certifications such as Alzheimer's or dementia care are a plus Experience working with individuals with developmental disabilities or disabilities is advantageous Ability to work independently while following care plans and protocols Strong communication skills for effective interaction with patients, families, and healthcare teams Ability to operate EMR/EHR systems efficiently for documentation purposes Valid driver's license preferred for travel between client locations Compassionate attitude with a focus on patient safety and dignity
