We are seeking Spanish speaking compassionate and dedicated Home Health Care Aides to provide exceptional in-home care to clients with diverse needs. This role involves supporting individuals in maintaining their independence, ensuring their safety, and enhancing their quality of life. The ideal candidate will have a strong background in patient care, excellent communication skills, and a genuine desire to make a positive difference in people's lives. Prior experience in long-term care, assisted living, or hospice settings is highly valued. This position offers the opportunity to work closely with clients requiring memory care, dementia support, and assistance with activities of daily living (ADLs), all while fostering a respectful and caring environment.
Duties
Assist clients with activities of daily living such as bathing, dressing, grooming, and mobility support
Monitor and observe patients' health status, including vital signs and overall condition
Administer medications and document medical records accurately using EMR/EHR systems
Support clients with meal preparation, feeding, and nutritional needs
Provide companionship and emotional support to promote mental well-being
Assist with light housekeeping tasks including laundry and cleaning to maintain a safe environment
Use specialized equipment such as Hoyer lifts or catheter care devices safely and effectively
Help with transportation to medical appointments or community activities when needed
Follow care plans meticulously while respecting patient privacy and confidentiality in compliance with HIPAA regulations
Document all care activities thoroughly in medical documentation systems
Skills
Proven experience in home health or long-term care environments; nursing home or assisted living experience is preferred
Knowledge of medical terminology, medication administration, and vital signs monitoring
Familiarity with EMR/EHR systems for accurate record keeping
Experience working with individuals with developmental disabilities, Alzheimer's disease, dementia, or memory care needs
Ability to assist with ADLs such as bathing, dressing, mobility assistance, and catheter care
CPR certification required; additional certifications like hospice care or working with people with disabilities are advantageous
Strong patient observation skills to identify changes in health status promptly
Compassionate caregiver capable of providing emotional support and companionship
Ability to work independently while following established care protocols and safety procedures
