Responsibilities
Assist clients with Activities of Daily Living (ADLs) such as bathing, dressing, grooming, toileting, and mobility support using equipment like Hoyer lifts when necessary.
Monitor and observe patients' health status by taking vital signs and reporting changes promptly to healthcare professionals.
Support medication administration following prescribed protocols while maintaining accurate medical documentation and adhering to HIPAA regulations for patient privacy.
Provide specialized care for clients with dementia or Alzheimer's disease, including memory care strategies and behavioral management techniques.
Assist with medical procedures such as catheter care, wound dressing changes, and hospice support as directed by healthcare providers.
Prepare nutritious meals and assist with feeding to ensure dietary needs are met while promoting healthy eating habits.
Maintain a clean and safe environment by performing light housekeeping tasks such as laundry, cleaning, and organizing to support overall well-being.
Document patient care activities accurately using Electronic Health Record (EHR) or Electronic Medical Record (EMR) systems to ensure seamless communication among healthcare team members.
Support clients with mobility devices like Hoyer lifts or wheelchairs safely during transfers and ambulation exercises.
Engage with clients compassionately, providing emotional support and companionship tailored to individual needs and preferences.
Requirements
Proven experience in home health care, assisted living facilities, nursing homes or long-term care environments.
Strong knowledge of medical terminology, patient observation techniques, and caregiving best practices.
Certification in CPR (Cardiopulmonary Resuscitation) is required; additional certifications such as medication administration or hospice care are a plus.
Experience working with people with developmental disabilities or dementia is highly desirable.
Ability to operate medical equipment like Hoyer lifts safely and effectively.
Familiarity with HIPAA regulations to ensure patient confidentiality at all times.
Excellent communication skills for working effectively with clients, families, and healthcare teams.
Flexibility to work live-in shifts or varied schedules based on client needs; valid driver's license preferred for community-based services.
