HOME HEALTH AIDE

For the Community of La Ronge

Lac La Ronge Indian Band Health Services is now inviting applications for 1 (One) Permanent Full Time, Home Health Aide for the community of La Ronge, Saskatchewan. The successful candidate will assist and provide Home Care and Diabetes Support services in the Home and Community Care Program for LLRIB community members.

Duties:

• Providing diabetic clients with case management, assessment and developing a goal based plan for care;
• Assist with the organization and coordination of wellness clinics;
• Provide diabetes education to effectively assist clients in maintaining an optimal level of independent functioning in relation to diabetes;
• Apply a knowledge-based Home Health Aide practice independently and in collaboration with the client and other health care professionals to promote independent living of clients;
• Must be able to provide home management, personal care and in-home respite assistance to clients;
• Use skills and interventions to meet the physical, social/cultural, emotional and spiritual needs of the client;
• Other duties as assigned by supervisor and/or designate;

Qualifications:

• Home Health Aide or Continuing Care Aide certificate;
• Willing to obtain further training related to Diabetes (provided by the employer)
• Must be able to work independently and collaboratively with the home care team;
• Must be self-directed and demonstrate organizational skills;
• Previous home care or long term care experience is an asset;
• Excellent verbal and written communication skills;
• Ability to speak or understand Cree is an asset;
• Must possess a Class 5 Driver's License;
• Criminal Record Clearance, Vulnerable Sector Screening and Driver's Abstract are required upon hire.
