Position:
To ensure the full implementation of the Home & Community Care Program Service Delivery Plan. To ensure the daily operations of the Home & Community Care Program meet the program goals and objectives while reflecting the Walpole Island First Nation philosophy, policies, and regulations, and the College of Nurses Ontario Regulations. Responsible for program and financial administration, and supervisory functions.
Requirements:
A. Experience
2 years' relevant work experience in a health care setting
Experienced and knowledgeable in working with First Nation People
Experience in a supervisory or coordinating role
Experience in financial administration and management
Experience in proposal writing is an asset
Experience with a quality assurance program is an asset
Experience with computerized data collection is an asset
B. Education
Minimum of BScN
Computer Literacy required
Psycho-Geriatric Assessment training or willing to obtain
Gerontology is an asset
Possess a current CPR and First Aid Certificate
C. Other
Possess the current College of Nurses of Ontario Registration and be a member in good standing with the RNAO (LAP insurance included)
Willing and able to work flexible hours as required
Must possess a valid Ontario Driver's license and reliable transportation
Knowledgeable of existing health and social services within the community and surrounding area, an asset
Must live and promote a healthy lifestyle
Subject to the criminal reference check policy
Responsibilities:
To coordinate and administer the initial and on-going development, implementation and evaluation of the Home & Community Program service delivery plan.
Review and comprehend the Home & Community Care Program service delivery plan and ensure full implementation.
To establish and implement a quality assurance plan for the Home & Community Care Program services.
To attend Health Committee meetings to provide monthly updates and obtain committee approval as needed.
To plan and initiate new and enhanced service delivery approaches on an on-going basis.
To coordinate and administer the Home & Community Care Program financial resources to ensure effective and efficient financial management and accountability.
To verify and negotiate the Home & Community Care Program home health support services contracts and protocol agreements.
To attend and participate in external meetings, information sessions, and planning groups that are relevant to this position.
To provide communication, public relations, liaison and professional resource support functions to the Health Committee, Chief & Council, community, and external government bodies.
Responsible for all aspects of personnel management of the Home & Community Care Program including supervision of coordinators and support staff: reviewing job descriptions; evaluations; identifying policy change requirements; related coordination of payroll and benefits.
To be directly accountable to the Director of Health and funding bodies, in accordance with Walpole Island Personnel Policy, Walpole Island Financial Regulations, Funding Agreement Terms and the Home & Community Care Program Service Delivery Plan.
To ensure establishment and maintenance of a Home & Community Care Program data base.
To provide initial and follow-up psycho-geriatric assessments for the Home & Community Care Program to determine and approve service needs.
To network with existing community health and social service providers within the community and surrounding area.
Maintain current knowledge of governmental guidelines and protocols which could affect the Home & Community Care Program.
