HOUR OF WORK: FULL-TIME, TEMPORARY (35 HRS/WEEK to JANUARY 2026)

WAGE: $25/hour
PROGRAM
The Health Department, in collaboration with the Tsawout First Nation (TFN) and broader community, is responsible for increasing the capacity and sustainability of the Department and enhance engagement/ownership of the health of the TFN community. The Department develops and implements health programs that incorporate the traditions, values and strengths of the Nation. The Health Department delivers a range of community health programs and health promotion services to the TFN community.
PURPOSE OF THE POSITION
The Home and Community Care Coordinator (HCC) is responsible for the coordination, delivery, and evaluation of home and community care services for Tsawout First Nation. The HCC ensures that care is delivered in accordance with client care plans, FNHA guidelines, and Tsawout policies. The role includes oversight of Health Care Assistants (HCAs), Homemakers, and other community care staff, while working closely with the Community Health Nurse (CHN) to support client wellness.
DUTIES AND RESPONSIBILITIES
KEY RESPONSIBILITIES
Program Coordination
Lead day to day operations of the Home and Community Care Program
Coordinate intake, assessment, and service planning for eligible clients in collaboration with the CHN
Assign and schedule homemaking and HCA visits based on client care plans and staff availability
Maintain accurate records of service delivery and ensure timesheets and task logs are submitted and filed
Manage and update the HCC Master Client List and Home Support Tracker
SUPERVISION & STAFF SUPPORT
Provide orientation, mentorship, and ongoing support to HCAs and Homemakers
Monitor and document staff attendance, timesheet accuracy, and daily checklists
Address coverage gaps, client refusals, and service adjustments with appropriate follow-up
Conduct regular 1:1 meeting with staff and complete performance check ins in collaboration with Health Director
Provide or arrange for training in areas such as documentation, safe work practices, and task delegation
POLICY & COMPLIANCE
Ensure services are delivered in alignment with FNHA's Home Care Service Delivery Guide and Tsawout policies
Maintain client confidentiality and uphold cultural safety in all interactions
Assist with the development or revision of procedures, staff job descriptions, and client documentation templates
Monitor worker classification (e.g., casual vs part-time vs full-time) to ensure alignment with payroll and contracts
CLIENT & FAMILY SUPPORT
Respond to client/family concerns, refusals, or non-responsiveness with empathy and documentation
Ensure client-centered care through individualized plans, regular check-ins, and culturally appropriate services
Implement tools such as daily visit logs, sign-in/out forms, and client task checklists
Carries out other duties essential to the position as directed by the Health Manager.
EDUCATION AND EXPERIENCE
Diploma or degree in health administration, community care, or equivalent combination of education and experience
Minimum 2 years of supervisory or coordination experience in a health or home care setting
Knowledge of FNHA Home Care policies, and Indigenous health practices preferred
Valid BC Driver's License and access to reliable transportation
Vulnerable Sector Criminal Record is a condition of employment

Excellent communication, organization, and time management abilities
Culturally safe, trauma-informed, and client-focused
Proficient in Microsoft Office and client tracking systems (Excel, EMR, etc.)
