Job Description

KSCS is seeking an experienced individual who is responsible to provide services to clients of HCCS on a flexible work schedule. Under the supervision of the Team Leader of Home Care and/or Assistant Team Leader of Home Care, the Home Health/Personal Care Aide is responsible to provide personal care to the clients of HCCS.


Department: Family Services, Home and Community Care Services (HCCS)


Duration: One (1) Year Contract with a 6-month probationary period


Salary: Starting at $25.32/hour


Schedule: Sporadic; must be willing to work flexible hours as necessary


Description




Under the supervision of the Team Leader of Home Care and/or Assistant Team Leader of Home Care, the Home Health/Personal Care Aide is responsible to provide personal care to the clients of HCCS.

Responsibilities




Provides personal home care services:

Provides personal and supportive care to clients. Provides medical assistance to clients. Under the supervision of the HCCS Nurse, administers daily medications at the TBEL, ILC and to clients at home (7 days per week). Assists client with bathroom needs. Provides specially authorized personal care. Prevents the spread of communicable disease. Provides household management and laundry. Provides for client nutritional intake. Organizes work according to client's needs and assigned tasks. Delivers "Meals on Wheels" (lunch Tuesday, and Thursday and Supper Monday thru Friday). Escorts clients to doctor's appointments. Provides transportation to and from clients home. Accompanies clients on errands i.e. bank, post, groceries. Provides and maintains a safe working environment for the client and Home Health Personal Care Aide. Ability to physically carry out the duties of the position

Provides assistance with Administrative Duties:

Attends meetings/workshops/conferences relevant to area of employment. Participates in weekly team meetings and planning sessions. Submits expenses, timesheets, and client statistic forms to immediate Supervisor on a monthly basis. Participates in regular supervision session with the immediate Supervisor. Participates in the yearly performance appraisal process with the immediate Supervisor.

Performs any other job-related duties as may be required by the immediate Supervisor.

Types of Schedule: + Changing schedule and hours.
+ Ability to work different shifts and flexible hours (days, evenings, weekends, 7:30 am to 8:00 pm, 7 days per week)
Stress Factor: Moderate stress to mostly high stress levels daily. Deadlines: Normal deadlines (planned).

Qualifications

Education and experience:



D.E.P. in Homecare and Family and Social Assistance Training or equivalent.

Skills and requirements:



Good interpersonal skills (works well with others). Knowledge of equipment pertaining to particular area is essential. Ability to work independently with minimal supervision and in a team setting. Must be familiar with customs, traditions and social issues of Kahnawa:ke Community. Experience in networking with other organizations and individuals. A valid drivers' license and access to a vehicle is a requirement. Ability to do a fair amount of local travel/car trips. Ability to work flexible hours, some overtime, early evenings and weekends. Ability to deal with moderate to high stress.

Assets:



Knowledge of Kanien'keha language. Knowledge of computers (Word, Excel).
