Description du poste :
Le coordonnateur ou la coordonnatrice de cas de soins \xc3\xa0 domicile est responsable de la coordination et de la prestation d\xe2\x80\x99un large \xc3\xa9ventail de services professionnels et non professionnels. Le coordonnateur ou la coordonnatrice de cas de soins \xc3\xa0 domicile est charg\xc3\xa9 de fournir un processus de collaboration ax\xc3\xa9 sur le client qui \xc3\xa9value, planifie, met en \xc5\x93uvre, coordonne, surveille et \xc3\xa9value les options et les services n\xc3\xa9cessaires pour r\xc3\xa9pondre aux besoins des clients en mati\xc3\xa8re de sant\xc3\xa9 et de services sociaux gr\xc3\xa2ce \xc3\xa0 l\xe2\x80\x99utilisation efficace et efficiente des ressources au sein des soins \xc3\xa0 domicile, de l\xe2\x80\x99Office r\xc3\xa9gional de la sant\xc3\xa9 d\xe2\x80\x99Entre-les-Lacs et de l\xe2\x80\x99Est et de la communaut\xc3\xa9. Le coordonnateur ou la coordonnatrice de cas de soins \xc3\xa0 domicile assure la liaison avec les organismes ou les \xc3\xa9tablissements qui s\xe2\x80\x99occupent des clients et de leurs proches aidants.

Le ou la titulaire du poste de coordonnateur ou de coordonnatrice de cas de soins \xc3\xa0 domicile exerce ses fonctions conform\xc3\xa9ment \xc3\xa0 la mission, \xc3\xa0 la vision et aux valeurs de l\xe2\x80\x99Office r\xc3\xa9gional de la sant\xc3\xa9 d\xe2\x80\x99Entre-les-Lacs et de l\xe2\x80\x99Est et les soutient.

Qualifications:

  • Un baccalaur\xc3\xa9at en travail social, un dipl\xc3\xb4me d\xe2\x80\x99ergoth\xc3\xa9rapeute, de physioth\xc3\xa9rapeute, d\xe2\x80\x99infirmier autoris\xc3\xa9 ou d\xe2\x80\x99infirmi\xc3\xa8re autoris\xc3\xa9e, d\xe2\x80\x99infirmier auxiliaire autoris\xc3\xa9 ou d\xe2\x80\x99infirmi\xc3\xa8re auxiliaire autoris\xc3\xa9e, de di\xc3\xa9t\xc3\xa9tiste ou un dipl\xc3\xb4me connexe en sant\xc3\xa9 ou en services sociaux est exig\xc3\xa9.
  • Une certification dans des programmes cliniques connexes serait un atout, p. ex., en g\xc3\xa9rontologie
  • Permis d\xe2\x80\x99exercer/enregistrement selon la d\xc3\xa9signation professionnelle requise et en r\xc3\xa8gle.
  • Si la candidate ou le candidat retenu est une ou un di\xc3\xa9t\xc3\xa9tiste autoris\xc3\xa9, elle ou il doit \xc3\xaatre dipl\xc3\xb4m\xc3\xa9 d\xe2\x80\x99un programme reconnu de stage reconnu en di\xc3\xa9t\xc3\xa9tique par les di\xc3\xa9t\xc3\xa9tistes du Canada.
  • Exp\xc3\xa9rience pr\xc3\xa9alable comme coordonnateur ou coordonnatrice de cas dans les services de sant\xc3\xa9 communautaires souhaitable.
  • Deux ans d\xe2\x80\x99exp\xc3\xa9rience dans le domaine des soins communautaires de pr\xc3\xa9f\xc3\xa9rence ou autre combinaison appropri\xc3\xa9e d\xe2\x80\x99exp\xc3\xa9rience pertinente pour le poste.
  • Exp\xc3\xa9rience de travail aupr\xc3\xa8s de personnes \xc3\xa2g\xc3\xa9es et de clients en soins palliatifs.

  • Connaissance ou exp\xc3\xa9rience approfondies des ordinateurs et du travail dans les dossiers de sant\xc3\xa9 \xc3\xa9lectroniques requise.
  • Capacit\xc3\xa9 \xc3\xa0 travailler de fa\xc3\xa7on autonome avec un minimum de supervision.
  • Capacit\xc3\xa9 \xc3\xa0 entretenir des relations de travail positives avec le personnel dans un milieu de travail fond\xc3\xa9 sur une \xc3\xa9quipe multidisciplinaire.
  • Permis de conduire valide du Manitoba et v\xc3\xa9hicule sont n\xc3\xa9cessaires.
  • Capacit\xc3\xa9 mentale et physique de r\xc3\xa9pondre aux exigences du poste.

Position Summary:
The Home Care Case Coordinator is responsible for the coordination and delivery of a broad range of professional and non-professional services. The Home Care Case Coordinator is responsible for providing a collaborative, client-driven process that assesses, plans, implements, coordinates, monitors and evaluates the options and services required to meet the client\xe2\x80\x99s health and human service needs through the effective and efficient use of resources within Home Care, Interlake-Eastern Regional Health Authority and the community. The Home Care Case Coordinator liaises with agencies or facilities involved with the clients and their family caregivers.

The position of Home Care Case Coordinator functions in a manner consistent with, and supports the Mission, Vision and Values of the Interlake-Eastern Regional Health Authority.
Qualifications:
  • BSW, OT, PT, RPN, RN, LPN, RD or related health/human service degree required.
    Certification in related clinical programs would be an asset, e.g. Gerontology
  • Licensure/registration as per professional designation required and in good standing.
    • If successful applicant is a Registered Dietitian, must be a graduate of a recognized dietetic internship program accredited by Dietitians of Canada.
  • Previous experience as a Case Coordinator in Community Health Services preferred.
  • Two years\xe2\x80\x99 experience in community-based care preferred or other suitable combination of experience relevant to the position.
  • Experience working with the elderly and palliative care clients.
  • Effective oral and written communication skills.
  • Experience with the interRAI Home Care (HC) Assessment System, Electronic Home Care Record, and Relias.
  • Strong knowledge/experience with computers and working within electronic health records required.
  • Ability to work independently with a minimum of supervision.
  • Ability to maintain positive working relationships with staff in a multi-disciplinary team based working environment.
  • Valid Manitoba drivers license and vehicle required.
  • Mental and physical ability to meet the demands of the position.
  • Completes and maintains a satisfactory pre-employment security check.
  • Satisfactory employment record.
