The HIV Treatment Nurse provides holistic patient and family-centric care and treatment support in collaboration with the treating Nurse Practitioner and the multidisciplinary team at Elevate NWO. This role liaises with community partners and provides related education and support, as necessary.
Accountabilities:
Review patient referrals from the Nurse Practitioner and the patient's eligibility for treatment.
Consult with the Nurse Practitioner on patient options and rationale for treatment.
Assess patient drug insurance coverage and assist with completing application forms when necessary.
Communicate regarding the various financial assistance or compensation programs available.
Support patients to obtain and maintain adherence to Medication (PrEP, PEP, ART)
Follow immediate intake protocols and ongoing monitoring.
Work collaboratively with the Nurse Practitioner and other healthcare providers to provide optimal patient care.
Perform venipuncture for consenting patients.
Plan a schedule of home visits and outreach appointments with patients to review and monitor treatment.
Involve and problem-solve with patients and educate on options for care and treatment.
Ensure the prescribing Nurse Practitioner receives regular follow-up information on patients and reports any crisis situations immediately.
Provide telephone support/care, as necessary.
Review immunization records & provide high-risk immunizations based on patient eligibility.
Develop a follow-up plan with patients.
Liaise with community partners to provide optimized health outcomes for patients.
Document each contact with patients or health care team, as per CNO requirements, in EMR, OCASE, and OCHART.
Report to the Ministry of Health as required.
Provide community education sessions as schedule permits.
Remain up to date on professional knowledge and evolving information.
Ad hoc duties as required.
Qualifications:
Registered Nurse, with a valid certificate of registration from the College of Nurses of Ontario (CNO).
Three to five (3 - 5) years of relevant experience; in a clinical setting is preferred.
Willing to participate in other professional bodies to enhance practice, e.g., Registered Nurses Association of Ontario and Canadian Association of Hepatology Nurses.
Community nursing experience is an asset.
Experience with chronic illness, mental health, substance use, and harm reduction would be a valued asset.
Demonstrated ability to provide services to a marginalized/stigmatized population in a social service, health care, or case management setting.
Experience with harm reduction approach with client interactions.
Must be comfortable working in an environment where discussions about sex, sexual expression, sexual orientation, and substance use are common given the work we do with our members/patients.
Able to adapt and be flexible in difficult situations.
Maintains discretion and a high level of confidentiality.
Must be able to pass the Vulnerable Persons Criminal Reference Check.
Must possess a valid Ontario driver's license and be willing to provide a clean driver's abstract.
The use of an insured motor vehicle to use for work-related purposes would be an asset.
