About the Opportunity
Reporting to the Manager of Seniors (Western Zone), or designate, the incumbent is a member of the Seniors Community Health Team and participates in the planning, development and provision of Social Work services to community-based seniors to enhance their independence and function at home. The Social Worker works to full scope of practice as an independent practitioner within the team. This includes supporting the development and integration of seniors\' health services within health care and the community and providing collaborative, client centered care across the continuum, supporting the psychosocial and emotional well-being of all individuals (includes patients, families, groups, populations and communities). This position includes considerable collaboration within the team and with community partners.
The Social Worker ensures that the principles and practices that guide NSHA such as the mission, vision, values, expected behaviors, and the leadership philosophy along with organizational health, population health, ethics, safety, quality, partnerships and interprofessional collaboration are integrated within the services one provides and through the messages one delivers.
