About the Opportunity
Reporting to the Manager of Dialysis or designate, the incumbent is a member of the Dialysis team and participates in the planning, development and provision of Social Work services to individuals receiving care within the Dialysis Unit. The Social Worker works to full scope of practice as an independent practitioner within the team. This includes supporting the development and integration of seniors' health services within health care and the community and providing collaborative, client centered care across the continuum, supporting the psychosocial and emotional well-being of all individuals (includes patients, families, groups, populations and communities). This position includes case finding activities and considerable community collaboration.
The Social Worker ensures that the principles and practices that guide NSH such as the mission, vision, values, expected behaviours, and the leadership philosophy along with organizational health, population health, ethics, safety, quality, partnerships and interprofessional collaboration are integrated within the services one provides and through the messages one delivers. Health Care Social Workers are actively involved in advocacy, education, program planning, consultation, and collaboration on behalf of individuals/groups within the institution.
