The Patient and Family Education program plays an integral role in supporting and promoting a culture of Health Literacy at Unity Health. This is achieved through programming, evaluation (of programming), reporting and continuous improvement related to best practice in patient/family education.
The primary role of the Health Promoter is to coordinate and implement health education/ promotion programs and facilitate integration of these programs in alignment with the UHT Health literacy focus. The health promoter works collaboratively with clinical teams, community groups and agencies to support program development and implementation.
The Health Promoter will focus on health literacy, health education and education interventions to assist patients and families of Unity Health. Responsibilities will include program development in partnership with clinical teams, subject matter experts, agencies and volunteers The Health Promoter position requires candidates to possess self-motivation, the capability of functioning independently or as part of a group, possess strong communication and leadership skills.
Duties and Responsibilities:
Liaise with internal clinical programs, clinician/subject matter experts as well as external organizations, including but not limited to community social support agencies, health & wellness based organizations e.g. Heart and Stroke, with a focus on enhancing/developing patients' and families' health literacy.
Develop networks (internally and externally) and work collaboratively with groups to strengthen and expand health promotion programming
Coordinate and communicate program initiatives across Unity Health Toronto
Provide brief consultations and expertise to other Unity Health providers and other organizations
Support the day to day operations of the Patient and Family Learning Centre (PFLC) through maintaining a strong volunteer program (in close partnership with Unity Health's volunteer coordinators) - including scheduling and coverage for volunteers, regular communication etc.
Support training of new PFLC volunteers
In partnership/direction with the manager, provide on-site support for the daily activities in the PFLC
Support the patient and family education program in management of the On-line health library
Work with the Collaborative Learning & Patient and Family Education program to ensure evaluation of programming
Using available software (e.g. Microsoft office suite, Survey Monkey) to support evaluation collection and analysis
Provide advice regarding the design of evaluation protocols.
Participate in evaluation data collection as needed.
Work schedule is typically weekdays with occasional early evenings
Qualifications:
Post-Secondary Education in Social Services from a recognized college or university in health promotion, public health, social science, or a related discipline.
Three to five years' experience in health promotion, public health, social service industry or related field
Excellent verbal and written communication. Listening, organizational, multi-tasking and problem-solving skills.
Experience and knowledge of program design, implementation and evaluation.
Demonstrated commitment to, and knowledge of, health literacy and resources.
Demonstrated ability to work effectively in an interprofessional team environment.
Ability to work with diverse stakeholders.
Willingness to be trained in detailed health literacy and best practices in patient education and to remain current.
The ability to take initiative and demonstrate flexibility and advanced problem solving skills, in order to work with complex client situations and to enthusiastically develop a unique health team position.
Experience in providing one-on-one supports (including assessment, problem-solving) to individuals, with diversity in ethnicity/language, income, education, ability, and mental health status.
Excellent verbal and written communication skills in English
Experience in building relationships with community partners that are outside the health care system.
Experience with interprofessional health teams an asset.
Experience in the development of educational materials targeted at patients and families.
Experience with data collection, research and quality improvement.
