Health Connector to join our team. The Health Connector supports the Manager of Health & Wellness in staff education and ensuring residents have access to necessary health services while advocating for their rights and needs within the community. The ideal candidate will be passionate about supporting individuals with disabilities, possess excellent communication skills, and be committed to positively impacting the lives of those we serve. This role includes resident assessment, staff education and training, advocacy, and resource navigation to ensure that all residents are supported in maintaining a healthy and fulfilled life.
Key Responsibilities:
Staff Education and Training:
Provide ongoing training and educational support to staff members on best practices for health and wellness, ensuring the integration of health-related services into daily care for residents.
Resident Care & Assessment:
Provide ongoing health assessments and support for residents, ensuring they have access to the necessary care and services. This position is open to multi-disciplinary professionals, if nursing background, Health Connector will assist residents with nursing functions.
Advocacy:
Advocate for residents to ensure they have access to necessary services and resources in the community, including healthcare, mental health services, and other essential programs.
Collaborative Support:
Work closely with the care team and external health professionals to coordinate services and ensure comprehensive care for residents.
Resource Navigation:
Assist residents, families and staff in navigating available services and resources, helping them to maintain or improve their health and well-being within the community setting.
Qualifications:
Experience in healthcare, nursing (LPN, RPN and RN), social work, or a related field (experience working with individuals with intellectual disabilities is considered an asset).
Strong communication and interpersonal skills, with the ability to advocate effectively on behalf of residents.
Knowledge of community resources and health services available to individuals with disabilities.
Ability to train and educate staff in health-related areas.
Compassionate, patient, and solution-oriented attitude towards supporting individuals with disabilities.
Saskatchewan Class 5 Driver's License with access to a vehicle.
