Responsibilities


Reporting to the Manager of Patient Flow, the Social Worker is a member of the interdisciplinary health care team providing collaborative, holistic care on the primary to quaternary care continuum. Social Workers are primarily concerned with the relationships between people and their environments which are integral to bio-psychosocial, emotional and spiritual well being. The social determinants of health are embedded in social work theory and practice and are the focus for the dynamic and interactive process of engagement, assessment, planning, intervention and evaluation. Social Workers build upon the strengths of patients/clients by focusing on existing coping strategies and problem solving skills, past successes, and personal resources.
The Social Worker ensures that the principles and practices that guide our work such as the mission, vision, values, expected behaviors, the leadership philosophy, organizational health, population health, ethics, safety, quality, partnerships and inter-professional collaboration are integrated within the services they provide and through the messages they deliver.

Qualifications


  • Bachelor of Social Work
  • Registration with the Nova Scotia Association of Social Workers
  • Ongoing professional development required
  • 2 years related clinical experience required
  • Recent related experience in an acute care setting an asset
  • Basic proficiency with Microsoft Office Suite (Word, Excel, PowerPoint, Outlook) and other electronic health programs such as Meditech, Medworxx, SEAscape, etc
  • Advanced knowledge and skills in:
  • theoretical frameworks and perspectives in one or more therapeutic modalities e.g., solution focused therapy
  • the social determinants of health and the risk factors and social consequences associated with diverse populations
  • theories of human behaviour, family systems and dynamics
  • conflict resolution skills
  • problem solving skills
  • medical terminology
  • knowledge of required diagnostic tools including the DSM
  • community resources and navigating systems
  • relevant legislation e.g. Protection of Persons in Care Act, Personal Directives Act, Privacy Act
  • cultural awareness and sensitivity
  • specific populations with uniquely defined needs and challenges, e.g., elderly, transplant
  • Demonstrated good attendance in current and past employment
  • Exemplary work history as demonstrated in current and past employment
  • Physical capabilities to perform the duties of the position
  • Travel may be required
  • Competencies in other languages an asset, French preferred

Hours of Work


Permanent full-time position; 37.5 hours bi-weekly. Some travel maybe required.

Salary Information


$33.30 - $42.05 hourly
